鼻内镜手术治疗非侵袭性真菌性上颌窦炎的临床观察

摘　　要：目的本研究诣在探讨鼻内镜下不同手术路径治疗非侵袭性真菌性上颌窦炎的临床效果。方法将本院耳鼻喉科收治的89例上颌窦炎患者随机分为观察组44例和对照组45例,观察组采用鼻内镜下中鼻道联合下鼻道开窗术治疗,对照组采用鼻内镜下上颌窦开放手术治疗。结果观察组的手术时间、术中出血量与对照组比较差异无统计学意义(P>0.05)。术前观察组与对照组的临床症状VAS评分比较,差异无统计学意义(P>0.05);术后第1个月、第3个月观察组的VAS评分低于对照组,且差异有统计学意义(P均<0.05)。术后第1个月、第3个月观察组的临床症状Lund-Kennedy鼻内镜黏膜形态评分分别为(1.25±0.59)分、(0.70±0.44)分均显著的低于对照组的(1.53±0.56)分、(0.98±0.42)分(P<0.05)。观察组的治愈率93.18%显著的高于对照组的75.56%(P<0.05)。结论鼻内镜下中鼻道联合下鼻道开窗术较鼻内镜下上颌窦开放手术治疗上颌窦的临床效果更好。Objective The purpose of this study was to explore different approaches under endoscopic operation to treat non invasive fungal maxillary sinusitis. Methods 89 cases of maxillary sinusitis patients in the Department of Otolaryngology of Hospital were randomly divided into the observation group（44 cases） and control group（45 cases）;observation group adopted nasal meatus jointly meatus fenestration treatment under nasal endoscope and the control group was treated with endoscopic maxillary sinus surgery. Results Operative time and blood loss of the observation group showed no significant differences from those of control group（P〉0.05）. Preoperative clinical VAS scores of the observation group and the control group showed no significant difference（P〉0.05）.The VAS scores of the 1st and 3rd months after operation of the observation group were significantly lower than those of the control group（P〈0.05）. The clinical symptoms Lund-Kennedy endoscopic mucosal morphology scores of the observation group at 1 month（1.25 ±0.59 points） and 3months（0.70±0.44 points） after operation were significantly lower than those of the control group（1.53±0.56 points,and 0.98±0.42 points, respectively）（P〈0.05）. The cure rate of the observation group（93.18%） was significantly higher than that of the control group（75.56%）（P〈0.05）. Conclusion Endoscopic sinus surgery under nasal endoscopy and endoscopic sinus surgery is better than that of endoscopic sinus surgery in the treatment of maxillary sinus.
