摘 要:目的观察沙格列汀并胰岛素泵治疗初诊2型糖尿病病人的效果。方法 2型糖尿病病人60例,随机分为A、B两组,每组30例。A组病人单纯应用胰岛素泵降糖治疗,B组病人应用胰岛素泵并沙格列汀降糖治疗,治疗2周。观察两组病人治疗前后空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、糖化清蛋白(GA)、空腹C肽及餐后2hC肽水平及胰岛素用量、血糖达标时间,同时计算低糖血症发生率。结果治疗后,两组FBG、2hPG、GA均较治疗前显著下降(t=5.50~13.94,P〈0.01);空腹及餐后2hC肽水平较治疗前显著升高(t=2.01~3.47,P〈0.05)。治疗后B组2hPG、GA低于A组,差异有显著性(t=3.33~5.66,P〈0.01);餐后2h C肽水平高于A组,差异有显著性(t=2.14,P〈0.05)。治疗后两组FBG、HbA1c水平比较差异无显著性(P〉0.05)。治疗后B组胰岛素用量少于A组,血糖达标时间短于A组,差异有显著性(t=3.10~6.34,P〈0.01)。结论沙格列汀并胰岛素泵治疗初诊2型糖尿病较单纯应用胰岛素泵可更好地控制FBG、2hPG,改善胰岛β细胞功能,减少胰岛素用量,缩短血糖达标时间。Objective To assess the efficacy of Saxagliptin combined with insulin-pump in the treatment of newly diag nosed type 2 diabetes mellitus (T2DM). Methods Sixty patients with newly diagnosed T2DM were divided into two groups as A and B in random, with 30 in each group. The patients in group A were treated with simple application of insulin-pump, those in group B were treated with Saxagliptin combined with insulin-pump, the course of treatment was two weeks for both groups. Fas- ting blood glucose (FBG), 2-hour postprandial glucose (2hPG), glycated hemoglobin (HbAlc), glycated album (GA), and C- peptide (C P) were measured in both groups, before and after therapy. The Insulin dosage, the days of blood glucose reaching standard level and the incidence of hypoglycemia were recorded and compared, Results After treatment, the findings in the two groups were as follows: FBG, 2hPG, and GA in both groups declined versus before treatment (t=5.50 13.94,P〈0.01) ; while fasting C P and 2 h C-P elevated (t=2.01-a.47,P%O.05) ; 2hPG and GA in group B were lower than that in group A (t =3.33- 5.66,P〈0.01), and 2 h CP was higher (t =2.14,P〈0.05). The difference in FBG and HbAle between the two groups was not significant (P〈0.05). Insulin used in group B was less than that in group A, and the time spent of blood sugar reaching the stan- dard level was shorter as well (t=3.10 6.34,P〈0.01). Conclusion Saxagliptin combined with insulin pump in the treatment of newly diagnosed type 2 diabetes is better than using simple insulin pump, the cotherapy can better control blood sugar and 2- hour postprandial blood sugar, improve islet β-cell function, reduce insulin dosage and shorten the time of blood glucose returning to stan-dard level.
