摘 要:目的观察M1、M2型巨噬细胞在小鼠周围神经损伤再生中的变化规律,探讨其对周围神经损伤再生的作用。方法将75只C57BL/6小鼠随机分为实验组、假手术组和正常对照组各25只。实验组游离坐骨神经后将其切断,行神经外膜端端吻合术,逐层关闭切口;假手术组仅游离出坐骨神经,然后关闭切口;正常对照组不做任何处理。于术后12h、24h、3d、7d、14d,每组每个时间点对5只小鼠术侧坐骨神经进行行取材。制成切片后分别行苏木精-伊红(HE)染色、免疫组化SABC染色,使用MIPS图像分析系统对切片中M1、M2型巨噬细胞数量进行统计,并分析与神经再生的可能关系。结果正常对照组在各个时间点HE染色均未见炎性细胞浸润,免疫组化染色均未观察到M1、M2型巨噬细胞。假手术组在各个时间点HE染色偶可观察到散在的淋巴细胞浸润,免疫组化染色可观察到少量的M1、M2型巨噬细胞。实验组HE染色见炎性细胞浸润渐增多,后期可见雪旺细胞大量增生,纤维组织增生明显;免疫组化染色示M1型巨噬细胞在12h、24h、3d及7d时大量增加,14d时有减少的趋势,M2型巨噬细胞数量在3d时开始明显增多,7d时继续增多,14d时达顶峰。结论周围神经损伤时,M1型巨噬细胞首先增殖,发挥促进神经再生的作用,M2型巨噬细胞后期增殖,可能抑制神经再生。两种巨噬细胞共同作用于周围神经损伤再生过程。Objective To investigate the law of changes of M1 and M2 type macrophages in regeneration of injured pe- ripheral nerves in mice, and explore the effect of the macrophages on regeneration of the nerves. Methods Seventy-five C57BL/ 6 mice were evenly randomized to three groups as experimental group, sham-operation group and normal control group. The sciatic nerve of mice in the experimental group was mobilized and cut off, end-to-end epineurial anastomosis was then done, the incision closed in layers; the nerve of mice in the sham opretion group was mobilized only, no any management was done for the mice in the control group. The sciatic nerve was taken for histology-12 h, 24 h, 3 d, 7 d and 14 d after surgery, five mice in each group for each time. All the sections were analyzed, after HE and SABC immuneohistochemistry staining, using MIPS imaging system, to add up the number of M1 and M2 type macrophages in the sections and analyze its possible correlation with nerve regeneration. Results For all the slices taken from the normal control group, no inflammatory cell infiltration and M1 and M2 type macrophages were noted. In sham-operation group, at each time point, scattered lymphocyte infiltration could be seen occationally in HE stai- ning slice, and a few M1 and M2 seen in SABC staining slice. In the experimental group: inflammatory ceils gradually increased, at later stage, schwann cells and fibrous tissue greatly increased. M1 type macrophages gradually increased in 12 h, 24 h, 3 d and 7 d, there was a trend of decrease in 14 d. For M2 type, the number started to increase in 3--7 d, reaching the peak in 14 d. Conclu- sion When peripheral nerves are injured, M1 type macrophages multiplicate first to educe nerve regeneration, and M2 multiplcate in late stage to probably inhibit nerve regeneration. The two kinds of the cells are involved in the process of neurotization of the in- jured nerves.
