慢性重型乙型肝炎患者的巨细胞病毒-pp65抗原与乙型肝炎病毒DNA、乙型肝炎e抗原及白细胞相关性研究

摘　　要：目的探讨慢性重型乙型肝炎患者CMV-pp65抗原与HBV DNA、HBe Ag及白细胞等因素的相关性。方法采用免疫组化二步法检测100例慢性重型乙型肝炎患者的CMV-pp65抗原,荧光定量PCR方法检测HBV DNA,酶联免疫吸附法测定HBe Ag,电阻抗法检测白细胞。结果 CMV-pp65抗原阳性组和阴性组HBV DNA病毒载量差异无统计学意义(Z=-1.185,P>0.05)。CMV-pp65抗原阳性组血清HBe Ag阳性14例,阴性组HBe Ag阳性31例,阳性组HBe Ag阳性率低于阴性组,差异有统计学意义(χ2=6.377,P<0.05)。CMV-pp65抗原阳性组血白细胞水平低于阴性组,差异有统计学意义(t=4.554,P<0.05)。结论 CMV-pp65抗原血症与血清HBe Ag阳性和白细胞水平呈负相关,与HBV DNA病毒载量无相关性。血清HBe Ag阴性和白细胞水平降低,可能是慢性重型乙型肝炎患者合并活动性CMV感染的危险因素之一。Objective To investigate the correlation of CMV- pp65 antigenemia in chronic severe hepatitis B patients with HBV DNA,HBe Ag and leukocyte level. Methods Immunohistochemistry two- step method was used to detect the CMV-pp65 antigen of 100 chronic severe hepatitis B patients,HBV DNA was detected by quantitative PCR,while HBe Ag was detected by enzyme- linked immunosorbent assay（ ELISA） and leukocyte count was detected by electrical impedance method. Results There was no statistical significance on difference in viral load of HBV DNA between positive group and negative group（ Z =- 1. 185,P〉0. 05）. 14 cases were with positive serum HBe Ag in the group of positive CMV- pp65 antigean,and so were 31 cases in the negative group. The positive rate of HBe Ag in the positive group was lower than the negative group,and the difference was statistically significant（ χ^2= 6. 377,P〈0. 05）. The leukocyte level in positive CMV- pp65 group was lower than that in the negative group,and the difference was statistically significant（ t = 4. 554,P〈0. 05）. Conclusion CMV- pp65 antigenemia had negative correlation with positive serum HBe Ag and leukocyte levels,while had no correlation with HBV DNA loads. Serum HBe Ag negative and low level of leukocyte may be one of the risk factors of chronic severe hepatitis B patients complicated with active CMV infection.
