不规则抗体与临床安全输血

摘　　要：目的总结本院不规则抗体阳性检出患者的抗体特异性及血液输注情况,保障临床用血安全。方法对全自动血型仪不规则抗体筛查阳性的样本,送市中心血站血液研究所做不规则抗体鉴定。再结合病历资料对临床用血情况及输注疗效进行综合分析。结果 23 473例患者样本中检出不规则抗体123例,阳性率为0.52%;其中非特异自身抗体阳性39例(占31.71%);特异性同种抗体阳性77例(占62.60%);自身抗体和同种抗体同时阳性7例(占5.69%)。61例不规则抗体阳性患者进行了输血治疗,除2例异体输血发生轻微过敏反应外,其余未发生输血不良反应,输血治疗有效。结论准确地进行不规则抗体筛查及特异性鉴定,选择合适的输血治疗方案,对于确保患者的输血安全和提高输血疗效具有重要的意义。Objective To summarize the specificity of the irregular antibody and the blood transfusion of patients with irregular antibody positive detection in our hospital,in order to ensure the safety of blood transfusion. Methods The positive samples examined by automatic blood analyzer for irregular antibody screening were sent to Ningbo Blood Center for identification of irregular antibody. The clinical blood using and the efficacy of transfusion were comprehensively analyzed,combined with the medical records. Results Among the 23 473 patient specimens,123 cases of irregular antibodies were detected,with the positive rate of 0. 52%. There were 39 cases of auto- antibodies（ 31. 71%）,77 cases of alloantibodies（ 62. 60%）,7 cases of autoantibodies and isotype antibodies（ 5. 69%）. Among them,61 cases of irregular antibody positive patients were treated with blood transfusion. The blood transfusion therapy was effective,in addition to 2 cases of allogeneic blood transfusion,in which slight allergic reaction occurred,and the rest did not show adverse transfusion reactions. Conclusion The screening and the specific identification of irregular antibodies are precisely performed,and selecting appropriate transfusion treatment plan has important significance for ensuring the safety of transfusion and improving the curative effect of blood transfusion.
