温脾疏肝汤联合西药治疗慢性乙型肝炎后肝硬化随机平行对照研究

摘　　要：[目的]观察温脾疏肝汤治疗慢性乙型肝炎后肝硬化疗效。[方法]使用随机平行对照方法,将60例住院及门诊患者按就诊顺序号随机分为两组;严格限制重体力活动及高强度体育锻炼,严格禁酒,进食易消化、产热少的食物,低盐低脂饮食。对照组30例拉米夫定,100mg/次,1次/d;氨基酸、维生素等营养支持。治疗组30例温脾疏肝汤(人参15g,干姜10g,柴胡9g,枳实15g,香附10g,茯苓12g,白术15g,陈皮12g,川芎10g,白芍12g,浙贝15g,炙甘草6g),1剂/d,水煎300m L,早晚口服;西药治疗同对照组。连续治疗15d为1疗程。观测临床症状、肝功能、肝纤维化、不良反应。连续治疗6疗程,判定疗效。[结果]临床疗效治疗组优于对照组(P<0.05)。AST、ALT、T-BIL、HA、LN、PCⅢ、C-Ⅳ两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。[结论]温脾疏肝汤治疗慢性乙型肝炎后肝硬化,疗效满意,无严重不良反应,值得推广。[Objective] To observe Wenpi Shugan Decoction therapy for Cirrhosis after Chronic Hepatitis B.[Method] With the way of Randomized parallel-group research, divide 60 patients with Cirrhosis after Chronic Hepatitis B into 2 group randomly. All patients were strictly limited on heavy physical activity and high intensity exercise, and prohibition against alcoholic drinks. And they must eat food which is easy to digest producing less Calori, and take Low salt,low fat diet. 30 patients in the control group were treated with Lamivudine pills（100mg PO Qd）, amino acids, vitamins and other conventional treatment. And 30 cases in treatment group were treated with Wenpi Shugan decoction（ginseng 15 g, dried ginger 10 g, radix bupleuri 9g, fructus aurantii immaturus 15 g, cyperus rotundus 10 g, tuckahoe 12 g, atractylodes 15 g, Tangerine Peel 12 g, ligusticum wallichii 10 g, radix paeoniae alba 12 g, zhebeinine 15 g, honey-fried licorice root 6g）, one potion a day, water frying 300 m L, oral in morning and evening, western medicine treatment is the same with the control group. Continuous treatment 15 d for one course.Observe their clinical symptoms,Liver function, Liver fibrosis, adverse reactions. Treat 6 courses of treatment Continuously and then evaluate efficacy. [Results] Clinical efficacy of the treatment group is better than the control group（P〈0.05）.The level of AST、ALT、T-BIL、HA、LN、PCⅢ、C-Ⅳ of all cases after treatment were relatively lower than before（P〈0.05）. The reducing degree of the treatment group was significantly higher than the control group（P〈0.05）.[Conclusion]Curative effect of Wenpi Shugan decoction for treating Cirrhosis after Chronic Hepatitis B is considerable,and no adverse reactions occurred, so it is worth Clinical promotion.
