消渴汤联合西药治疗糖尿病肾病随机平行对照研究

摘　　要：[目的]观察消渴汤联合西药治疗糖尿病肾病疗效。[方法]使用随机平行对照方法,将50例住院及门诊患者按掷骰子法简单随机分为两组。对照组25例二甲双胍,0.5g/次,3次/d,慢性肾病3a期减量,内生肌酐清除率〈50m L/min停二甲双胍;慢性肾病3-4期用格列美脲,1-2mg/次,1次/d,起始剂量1mg/d;羟苯磺酸,1g/片,3次/d。治疗组25例消渴汤（党参10g,生黄芪20g,白术-炒10g,金樱子20g,芡实、白术、茯苓、山药各10g,黄精、菟丝子各20g,百合、枇杷叶各10g）,1剂/d,水煎400m L,早晚口服;西药治疗同对照组。连续治疗70d为1疗程。观测临床症状、FPG、2h PG、HbAlC、Scr、UAE、BUN、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效16例,有效8例,无效1例,总有效率96.00%。对照组显效7例,有效12例,无效6例,总有效率76.00%。治疗组疗效优于对照组（P〈0.05）。治疗组生化指标均有改善（P〈0.01）,对照组FPG无改善（P〉0.05）,其余均有改善（P〈0.01）,治疗组改善优于对照组（P〈0.05）。[结论]消渴汤联合西药治疗糖尿病肾病疗效满意,无严重不良反应,值得推广。[Objective]To observe the decoction combined with western medicine diabetes diabeticnephropathy.[Methods] Randomized parallel controlled method,50 inpatients and outpatients by dice method is simple randomized into two groups.The control group of 25 patients with metformin,0.5g/time,3 times/d,3a of reduced chronic kidney disease,endogenous creatinine clearance〈50m L/min stop metformin;3 to 4 chronic kidney disease with glimepiride,1-2mg/times,1 times/d,the initial dose of 1mg/d;dobesilate,1g/tablets,3 times/d.The treatment group 25 cases Diabetes soup（Dangshen 10 g,Shenghuangqi 20 g,Baizhu 10 g,Jinyingzi 20 g,Qia nshi,Baizhu,Fuling,Shanyao 10 g,Huangjing,Tusizi 20 g,Baihe,Pipaye each 10g）,1 ji/ d,decoction 400 m L,sooner or later,oral,200 m L/times.Western medicine with the control group.70 d for a course of continuous treatment.Observation of clinical symptoms,FPG,2h PG,Hb Al C,Scr,UAE,BUN,adverse reactions.One course of treatment,to determine efficacy.[Results] The treatment group 16 cases,effective in 8 cases,one case,the total efficiency of 96.00%.Group 7 cases markedly effective in 12 cases,6 cases,the total efficiency of 76.00%.Treatment group than the control group（P〈0.05）.Biochemical indicators have improved treatment group（P〈0.01）,FPG no improvement in the control group（P〉0.05）,the rest were improved（P〈0.01）,the treatment group than the control group improved（P〈0.05）.[Conclusion] Diabetes soup combined with Western medicine treatment of diabetic nephropathy satisfaction,no serious adverse reactions,is worth promoting.
