妇产科医院抗菌药物临床应用专项管理成效分析

摘　　要：目的探讨抗菌药物专项整治前后医院抗菌药物临床应用情况和管理成效评价。方法统计医院信息系统中2010—2014年抗菌药物临床应用数据,依据《2011年全国抗菌药物临床应用专项整治活动方案》所提出的指标进行回顾分析和总结。结果抗菌药物使用金额占比、门诊抗菌药物处方占比、住院患者抗菌药物使用率和Ⅰ类切口手术预防抗菌药物使用率分别由整治前(2010年)的18.53%、16.54%、73.53%和67.17%下降至整治后(2014年)的3.75%、8.82%、41.25%和11.88%,符合国家卫计委规定值,下降趋势均具有统计学意义(P=0.000)。其他各项指标如抗菌药物使用强度、接受抗菌药物治疗患者微生物检验标本送检率等指标均达到国家卫计委要求。结论通过开展抗菌药物专项整治活动使医院抗菌药物临床应用更加合理,管理成效明显。Objective To investigate clinical antibiotic use and assess management efficacy in obstetrical and gynecological hospitals before and after the special antibiotic rectification.Methods The data of clinical antibiotic use in the hospital information system from 2010 to 2014 were retrieved and retrospectively analyzed and summarized according to indexes proposed by the 2011 National Special Rectification Program for Clinical Antibiotic Use.Results The ratios of the amount of used antibiotics to the total amount and clinical antibiotic prescriptions to total prescriptions and antibiotic use rates in inpatients and type I incision operations decreased from 18.53%,16.54%,73.53%and 67.17%before rectification（2010） to 3.75%,8.82%,41.25%and11.88%after rectification（2014）,respectively,which met the requirements of the National Health and Family Planning Commission.The decline tendency was statistically meaningful（P = 0.000）.Other indexes such as antibiotic use density（AUD） and sample test rates for patients received antibiotic therapy were also achieved the requirements of the National Health and Family Planning Commission.Conclusion The clinical antibiotic use is more reasonable and the management efficacy is significant via the special antibiotic rectification.
