合肥市基层医疗机构检验科妇幼卫生项目检测能力现况分析

摘　　要：目的了解合肥市乡镇卫生院、社区卫生服务中心实验室检测能力及质量管理现状,评估基层医疗机构承担妇幼卫生项目的能力,为制定基层医疗机构实验室能力建设方案提供参考依据。方法采取随机抽样方法选取43家乡镇卫生院和社区卫生服务中心,对其检验科进行现场调查和室间比对。结果实验室业务用房面积平均44.24 m2;仪器设备以国产为主,58.08%的设备使用时间超5年;人员配置偏少(平均2.72人/所),专业技术人员以初、中级职称和大专及以下学历为主,非检验专业人员占18.80%;实验室管理制度不健全,质量管理水平较低;室间比对结果合格率较低(血常规41.86%、临床生化39.53%、HBs Ag 86.05%、梅毒88.37%、HIV抗体90.70%)。结论加大对实验室设备、业务用房的投入,加快人才队伍建设,强化实验室质量管理,从而加强基层医疗机构实验室能力建设,全面提升基层医疗机构检验水平,以保证妇幼卫生项目的检验质量。Objective To understand the current situations of detection ability and quality management of clinical laboratory in township health centers and community health service centers in Hefei city, evaluate the ability of basic medical institutions for maternal and child health care project, provide reference basis for making construction scheme of laboratory capacity in primary medical institutions. Methods Forty-three township health centers and community health service centers were selected by random sampling method, field investigation and interlaboratory comparison were conducted among the clinical laboratories. Results The average area of laboratory was 44. 24 m2 ; domestic equipments accounted for most of the equipments, 58.08% of the equipments were used over 5 years; the personnel allocation was low （2. 72 people every institution on average） , the most professional qualifications were primary and intermediate, and most of the academic degrees were college and below; 18.80% of the staff was not laboratory medical professionals; laboratory management system was not integrated, the quality management level was low ; the qualification rates of interlaboratory comparison Were low （ blood routine test ： 41.86% ; clinical biochemistry： 39.53% ; HBsAg： 86. 05% ; syphilis： 88.37% ; HIV antibody： 90. 70% ） . Conclusion Laboratory capacity of basic medical institutions should be strengthened by increasing input into laboratory equipment and office space, cultivating talents team and managing laboratory quality. Laboratory quality level should be improved to ensure laboratory detection results of maternal and child health care project.
