解毒通瘀复肾颗粒联合西药治疗血瘀型IgA肾病随机平行对照研究

摘　　要：[目的]观察解毒通瘀复肾颗粒联合西药治疗血瘀型IgA肾病疗效。[方法]使用随机平行对照方法,将36例住院患者按掷骰子法简单随机分为两组;肾上腺皮质激素、转换酶抑制剂、缓解蛋白尿等。对照组18例贝那普利,10mg/次,1次/d。治疗组18例解毒通瘀复肾颗粒(牛膝30g,山茱萸15g,黄芪60g,路路通、赤芍、车前子各15g,大黄6g,党参30g),真空浓缩、喷雾干燥后制成颗粒,10mg/次,1次/d;西药治疗同对照组。连续治疗30d为1疗程。观测临床症状、尿RBP、尿α_1-MG、尿β_2-MG、血小板源性生长因子BB、不良反应。治疗1疗程,判定疗效。[结幂]治疗组痊愈3例,显效5例,有效9例,无效1例,总有效率94.44%。对照组痊愈1例,显效4例,有效7例,无效6例,总有效率66.67%。治疗组疗效优于对照组(P<0.05)。尿检指标两组均有改善(P<0.01),治疗组改善优于对照组(P<0.05)。血小板源性生长因子BB指标两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]解毒通瘀复肾颗粒联合西药治疗血瘀型IgA肾病疗效满意,无严重不良反应,值得推广。[Objective]To observe the detoxification Tongyu fushen particles even worsen western medicine and blood stasis IgA nephropathy.[Methods]Randomized parallel controlled method,36 cases of hospitalized patients were randomly divided into two groups craps method is simple.Adrenocorticotropic hormone,converting enzyme inhibitors,remission of proteinuria treatment.18 cases in the control group benazepril,10mg/times,l times/d.Treatment group,18 patients detoxify Tongyu fushen particles（Niuxi 30 g,Shanzhuyu 15 g,Huangqi60g,Lulutong,Chishao,Cheqianzi 15 g,Dahaung 6g,Dangshen 30g）,concentrated in vacuo,spray-dried into granules,10mg/times,l times/d;western medicine with the control group.30 d for a course of continuous treatment.Observation of clinical symptoms,urine RBP,urinary 1-MG,urinary 2-MG,platelet-derived growth factor BB,adverse reactions.One course of treatment,to determine efficacy.[Results]The cure 3 cases,5 cases markedly effective in 9 cases,one case,the total efficiency of 94.44%.Cured one case in the control group,effective in 4cases.effective 7 cases,6 cases,the total efficiency of 66.67%.Treatment group than the control group（P〈0.05）.Urine indicators improved in both groups（P〈0.01）,the treatment group than the control group improved（P〈0.05）.Platelet-derived growth factor BB index improved in both groups（P〈0.01）,the treatment group than the control group improved（P〈0.01）.[Conclusion]detoxification Tongyu fushen particles combined with Western medicine and blood stasis satisfied with IgA nephropathy,no serious adverse reactions,is worth promoting.
