含透明质酸的保湿护肤品治疗面部脂溢性皮炎的临床研究

摘　　要：目的评估含透明质酸的保湿护肤品(简称:保湿霜)治疗面部脂溢性皮炎(seborrheic dermatitis,SD)的临床疗效、对皮肤屏障功能的影响和安全性。方法 98例SD患者接受保湿霜治疗28d,2次/d。在治疗第0,7,14,28天由皮肤科医生对患者疗效及安全性、患者的满意度进行评估,并测定患者皮肤屏障功能相关指标。结果治疗28d后,患者皮损严重程度评分降低(87.37±15.18)%,94.90%的患者对疗效自评达显著好转或治愈。其中治疗7d后,患者严重程度评分降低(59.84±17.37)%,而52.03%的患者对疗效自评达显著好转或治愈。治疗后,患者皮脂含量从(72.92±21.04)μg/cm2升高至(85.67±19.78)μg/cm2,皮肤平均水含量也从(71.03±2.98)%升高至(73.61±3.13)%。经皮失水(TEWL)从(7.32±1.24)g/(m2.h)降低至(6.44±1.18)g/(m2.h),皮肤的酸碱度和弹性,黑素及血红素含量保持稳定。治疗过程中,只有7例(7.14%)患者有轻微的刺激反应,无1例患者因不良反应而中断治疗。结论保湿霜治疗SD的临床疗效好,且安全性高。Objective To evaluate the clinical efficacy, safety and skin barrier function of facial seborrheic dermatitis （SD） treated with a moisturizer containing hyaluronan. Methods Ninety - eight patients with SD were enrolled in this study. The patients received treatment of the moisturizer twice daily for 28 days. The severity of the lesions , patients＇satisfaction , therapeutic safety and skin barrier function were evaluated and scored by a dermatologist on day 0, 7, 14 and 28, respectively. Results After 28 days of treatment, the severity scores of the SD patients decreasing by 87.37 ± 15.18% （P 〈0.05）. 94.90% patients experienced major improvement or complete clearance of SD. The improvement was most significant during the first week of treatment with the scores of the SD patients decreasing by 59.84 ± 17.37% （ P 〈 0.05 ）, while 52.03% patients＇self-assessment showed significant improvement or cure. After treatment, the skin sebum content of SD patients were increased from 72.92 ±21.04 μg/cm^2 to 85.67 ± 19.78 μg/cm^2 （P 〈 0.05 ）. The average hydration were also increased from 71.03 ± 2.98 % to 73.61 ± 3.13% （P 〈 0.05）. Transdermal water loss （TEWL） was reduced from 7.32 ± 1.24g/m^2. h to 6.44 ± 1. 18g/m^2. h （P 〈0.05）. The skin pH, elasticity, melanin and hemoglobin levels remained fairly stable. During the treatment, only 7 （7.14%） patients shown mild irritation. No patient discontinued the treatment due to the side effects. Conclusion The moisturizer have significant therapeutic effects for facial seborrheic dermatitis without significant side effects.
