日本血吸虫病肝域声速值检测方法探讨

摘　　要：目的:应用声速组织定量技术(sound velocity tissue quantification,STQ)测定早期日本血吸虫感染者肝脏域速度指数(zone speed index,ZSI),获得域声速值(zone sound speed,ZSS),并探讨检测方法。方法:应用ZONARE Zone Ultra超声仪检测肝纤维化声像图0级血吸虫病肝39例,分别检测肝左叶包膜下0、1cm,肝右叶包膜下0、1、2、3cm处ZSI值,计算各部位检测成功率、组内相关系数(ICC),选择合适检测部位。分析年龄、性别及检测深度与ZSS值的关系。结果:血吸虫病肝ZSI值检测稳定性较好,与年龄、性别不相关,肝右叶包膜下1cm为最佳检测深度。结论:声速组织定量技术可以应用测定肝脏ZSI值定量评估血吸虫病肝组织硬度。Objective:To explore detections for the early infected persons of schistosoma japonicum by means of sound speed tissue quantification(STQ)to get liver zone speed index(ZSI)and then obtain zone sound speed(ZSS).Methods: With STQ,Zonare Zone Ultra had been applied to detect hepatic fibrosis in the 39 cases of zero-level schistosomiasis.The ZSI values at 0cm or 1cm below liver capsule in the left lobe,and at 0,1,2or 3cm in the right had been detected.The achievement rate at each position had been calculated with intra-group correlation coefficient(ICC)to select suitable positions for detection.The relationship between depth,age or sex and the ZSS values had been analyzed.Results:Measuring liver ZSI with STQ was of high stability.The ZSI values of schistosomiasis liver were not correlated with age and sex.The achievement rate at 1cm below liver capsule of right lobe was the highest.Conclusion:ZSI measured with STQ can be used to assess the schistosomiasis liver elasticity quantitatively.
