摘 要:目的探讨抗反转录病毒治疗(ART)前后,艾滋病病毒1型(HIV-1)感染者外周血中调节B细胞(Breg)和记忆B细胞(Bmem)的平衡,以及白介素-10(IL-10)表达水平与HIV-1感染疾病进展的关系。方法选择30例未治疗的HIV-1感染者,于ART治疗前和治疗12周,分离外周血单个核细胞(PBMC),用流式细胞术(FCM)检测CD19+CD24hi CD38hi B细胞、CD19+CD24hi CD38loB细胞的及CD19+IL-10+B细胞比例的变化;用酶联免疫吸附试验法(ELISA)测定培养上清中IL-10水平,与健康对照组比较。结果相对于健康对照组,感染HIV-1后,CD19+CD24hi CD38hi B细胞的比例(P=0.0001)、Breg/Bmem比值(P<0.0001)及上清中IL-10水平(P=0.0028)均升高,而CD19+CD24hi CD38loB细胞的比例(P=0.0005)显著降低,差异具有统计学意义。ART开展12周后,CD19+CD24hi CD38hi B细胞的比例(P=0.0069)、Breg/Bmem比值(P=0.0012)及上清中IL-10水平(P=0.041)均显著降低,且上清中IL-10水平与Breg(R2=0.1347,P=0.0039)的比例呈正相关性,而与Bmem细胞的比例呈显著负相关(R2=0.1705,P=-0.001)。结论 HIV-1感染改变了CD19+CD24hi CD38hi B细胞的比例、其相关细胞因子IL-10的表达水平,并破坏了Breg/Bmem平衡;经过ART治疗后,CD19+CD24hi CD38hi B细胞的比例与其相关细胞因子IL-10的表达水平及Breg/Bmem平衡可以得到逐渐恢复。Objective To study the balance between regulatory and memory B cells and concentration of IL-10 in the supernatant of cultured PBMC in different disease progression of Chinese HIV-1infected individuals before and after antiretroviral therapy(ART)was initiated.Methods The changes of regulatory B(Breg,CD19+CD24hi CD38 hi B)and memory B(Bmem,CD19+CD24hi CD38loB)subsets in peripheral blood were determined by flow cytometry(FCM)and the levels of IL-10 in the supernatant were measured by ELISA in ART-nave HIV infected group,ART group and healthy controls.Results Breg frequency(P= 0.0001),Breg/Bmem(P 〈0.0001)and IL-10concentration(P= 0.0028)were higher in ART-nave HIV infected group than those in healthy controls,with Bmem(P= 0.0005)diminished.This imbalance was summarized by the Breg/Bmem ratio.However,the Breg/Bmem ratio presented restoration after ART for 12 weeks.The concentration of IL-10 was positive with Breg frequency in HIV-1infected patients(R2= 0.1347,P= 0.0039).Conclusion The expression of CD19+CD24hi CD38 hi B subsets and related cytokines IL-10 are increasing due to HIV infection,with imbalance between Breg and Bmem B cells,which can be gradually restored by ART treatment.
