基于NIHSS评分的分组护理对脑出血患者术后神经康复和生活质量的影响

摘　　要：目的:探讨基于美国国立卫生研究院卒中量表(national institutes of health stroke scale,NIHSS)评分的分组护理对脑出血患者术后神经康复和生活质量的影响。方法:选取延安大学附属医院于2014年2月至2015年2月收治的120例脑出血患者为研究对象,根据NIHSS评分将患者分为≥10分组(60例)和<10分组(60例)。两组根据NIHSS评分分别给予针对性的预见性护理措施,比较两组患者术后神经康复和生活质量的情况。结果:≥10分组的总有效率显著高于<10分组,差异具有统计学意义(χ2=11.582,P=0.001)。≥10分组的神经功能康复程度高于<10分组,且两组患者间的NIHSS评分差异有统计学意义(P<0.05)。≥10分组的活动能力、家庭角色、思维、情绪、社会角色及肢体功能评分均显著高于<10分组(P<0.05)。结论:基于NIHSS评分对脑出血患者进行分组护理,给予针对性的预见性护理措施,能够合理分配和利用护理资源,减少患者发生并发症,改善患者的神经功能恢复,提高患者的生活质量,促进患者早日康复。Objective： To discuss the influence of grouping nursing based on national institutes of health stroke scale（ NIHSS） score on the recovery of neural function and the quality of life in patients with cerebral hemorrhage.Methods： One hundred and twenty cases with cerebral hemorrhage were divided into two groups according to NIHSS scores made at admission： NIHSS≥10 group（ group Ⅰ） and NIHSS 〈10 group（ group Ⅱ）. The group Ⅰreceived normal nursing,while the group Ⅱ received early rehabilitation nursing. The recovery of neural function and the quality of life of the two groups were compared. Results： The total effective rate in group Ⅱ was significantly higher than that of the group Ⅰ with statistical difference（ χ^2= 11. 582,P = 0. 001）. The recovery of neural function in group Ⅱ was significantly better than that of the group Ⅰ with statistical difference（ P〈0. 05）.The quality of life,including physical function,family role,cognitive function,emotional function,social function and the overall quality of life,was significantly improved in the group Ⅱ（ P〈0. 05）. Conclusion： Grouping nursing based on NIHSS score can improve the recovery of neural function and the quality of life,and reduce the occurrence of complications in patients with cerebral hemorrhage.
