摘 要:目的探讨甲状腺影像报告和数据系统(TI-RADS分类)在诊断不同直径甲状腺结节中的应用价值。方法经病理证实的258例(316个结节)甲状腺结节患者,按直径的不同分为≤10mm组,10~20mm组,≥20mm组,采用TI-RADS分类系统对3组甲状腺结节进行回顾性分析,同时分析各个恶性征象。结果 TI-RADS分类4b类及以上考虑恶性,三组甲状腺结节的约登指数和准确度分别为:0.621、0.797、0.643和87.2%、90.1%、84.6%;边缘不规整在≤10mm组及≥20 mm组中有极强关联,纵横比≥1在≤20 mm组中具有极强关联,边界不清、低回声、微钙化在三组中具有强关联。结论 TI-RADS分类系统在直径10~20mm组中的诊断价值最高;可着重观察不同直径结节表现出较明显的恶性征象提高诊断的准确度。Objective To explore the applied value of thyroid imaging reporting and data system(TI-RADS)classification in diagnosing different diameter thyroid nodules.Methods Three hundred and sixteen histologically proved thyroid nodules in 258 patients were divided into three groups—10mm or less,10~20mm and 20 mm or higher,according to the diameter of thyroid nodules.Three groups of thyroid nodules were retrospectively analyzed by the TIRADS classification,and the malignant signs would be analyzed at the same time.Results TI-RADS classification of 4b and above was considered as malignant.The Youden′s index and diagnostic accuracy of the three groups were 0.621,0.797,0.643 and 87.2%,90.1%,84.6%;margin not circumscribed has a very strong correlation in groups of 10 mm or less and 20 mm or higher,while taller than wide≥1has a strong correlation in group of 20 mm or less.Conclusions TI-RADS classification has the highest valuation in diagnosing group of 10~20mm.It′s suggested that we can observe different diameter on nodules which showed the most obvious signs of malignant to improve the accuracy of diagnosis.
