摘  要:目的应用实时三维经食管超声(RT-3DTEE)分析二尖瓣脱垂患者二尖瓣瓣环立体结构在全心动周期的动态变化特征。方法 57例行RT-3DTEE检查,对照组33例,二尖瓣脱垂患者(MVP组)24例。应用二尖瓣定量分析软件于舒张早、中、晚期(ED,MD,LD),收缩早、中、晚期(ES,MS,LS)测量二尖瓣瓣环。结果 (1)MVP组DAP于ED最大,DAlPm至LD最大,逐渐减小至ES,H于LD至LS升高,H/DAlPm至MS最大,E2D至ES最大。(2)与舒张期相比,MVP组DAP于MS明显减低,H、H/DAIPm于LS明显增大,E2D于MS明显增大(P<0.05),而DAlPm各期间比较差异均无统计学意义(P>0.05)。结论 MVP瓣环是动态变化的,收缩期仍保留"马鞍形"结构,但运动幅度增大。Objective To analyze the mitral annulus geometry and dynamic over the cardiac cycle in patients with mitral valve prolapse by real-time 3Dtransesphageal echocardiography(RT-3DTEE).Methods RT-3DTEE of the mitral valve was acquired in 57 subjects,including 33 normal subjects,24 patients with mitral valve prolapse(MVP group).Mitral annular measurements were performed during the cardiac cycle in early,middle,and late diastole(ED,MD,LD)and early,middle,and late systole(ES,MS,LS)using Mitral-Valve-Quantification Software.Results(1)In MVP group,DAP reached peak at ED.DAlPm reached the minimum at ED,then gradually increased,and reached peak at LD.H increased from LD to LS.H/DAlPm reached peak at MS.E2 Dreached peak at ES.(2)Compared with diastole in MVP group,DAP was significantly decreased at MS.H and H/DAlPm were significantly increased at LS.E2 Dwas significantly increased at MS(P〈0.05).DAlPm has no significant difference among each period(P〉0.05).Conclusions MVP annulus is dynamic throughout the cardiac cycle and remains "saddle" structure at systolic with its range of motion increased.
