超声与MRI产前诊断凶险性前置胎盘的临床意义

摘　　要：目的探讨凶险性前置胎盘超声及磁共振成像(MRI)产前诊断的价值。方法纳入对象为本院产后确诊的102例凶险性前置胎盘的孕妇,回顾性分析每位孕妇产前超声及MRI图像,计算超声及MRI诊断的准确度。根据诊断结果是否一致分为一致性组及不一致性组。卡方检验比较两组孕产妇分娩方式、手术处理方式及输血量是否存在显著性差异。结果超声与MRI诊断结果一致时,孕产妇择期剖宫产率较高。超声与MRI诊断结果不一致时,孕产妇紧急剖宫产率较高。而两组孕产妇手术处理方式及输血量无显著性差异。结论超声与MRI在产前诊断凶险性前置胎盘中均具有重要的临床意义,其诊断结果直接影响临床医师对孕产妇的处理方式。Objective The aim of this study was to determine the value of prenatal ultrasound and MRI in diagnosis of pernicious placenta previa（PPP）.Methods 102 cases diagnosed as PPP were studied.Calculate the accuracy of diagnosis in ultrasound and MRI.According to the diagnostic results,all cases were divided into the same consistency group and the inconsistency group.Compare whether there were significant differences in the way of delivery,surgical treatment and blood transfusion between the group of consistence and inconsistence in ultrasound and MRI by Chi-square test.Results When the diagnostic results of ultrasound and MRI are consistent,the rate of elective cesarean section is high.When the diagnostic results are inconsistent,the rate of emergency caesarean section is high.There were no significant differences in the way of surgical treatment and blood transfusion.Conclusions Ultrasound and MRI have also important clinical significances in diagnosis of PPP,which directly affect the way of treatments.
