摘　　要：目的 观察ω-3鱼油脂肪乳对感染性休克患者早期炎症指标的影响.方法 选择2013年1月至2015年1月解放军第一○○医院收治的感染性休克患者40例,按照区组随机法分为观察组和对照组,各20例.2组分别在常规治疗的基础上静脉注射ω-3鱼油脂肪乳100 ml和0.9％氯化钠注射液100 ml,治疗1周后观察2组患者C反应蛋白（CRP）、降钙素原、肿瘤坏死因子α（TNF-α）、白细胞介素6（IL-6）的变化结果 治疗1周后,2组患者CRP、降钙素原、TNF-α、IL-6均较治疗前明显下降[对照组：（58±12） mg/L比（120±13） mg/L,（10±4）μg/L比（21±4）μg/L,（42±15） ng/L比（70±18） ng/L,（255±66） μmol/L比（302±66） μmol/L;观察组：（35±12）mg/L比（131±13）mg/L,（5±4） μg/L比（20 ±5） μg/L,（21±15） ng/L比（70±19）ng/L,（201±65） μmol/L比（296±68） μmol/L],且观察组明显低于对照组,差异均有统计学意义（均P＜0.05）.结论 感染性休克患者早期应用ω-3鱼油脂肪乳可降低患者全身炎性反应强度.Objective To investigate the effect of ω-3 fish oil fat emulsion on inflammatory indices in patients with early stage septic shock.Methods Totally 40 patients with septic shock from January 2013 to Janual 2015 were randomly divided into observation group （20 cases） given intravenous injection of ω-3 fish oil fat emulsion on the basis of conventionial therapy,and control group （20 cases） given intravenous injection of 0.9% sodium chloride on the basis of conventionial therapy.One week after treatment,the levels of C-reactive protein （CRP）,procalcitonin （PCT）,tumor necrosis factor-α （TNF-α）,interleukin-6 （IL-6） were compared between two groups.Results After 1 week of treatment,CRP,PCT,TNF-α,IL-6 were significantly reduced compared with those before treatment in control group [（58 ± 12） mg/L vs （120 ± 13） mg/L,（10 ±4） μg/L vs （21 ±4） μg/L,（42 ± 15） ng/L vs （70 ± 18） ng/L,（255 ±66） μmol/L vs （302 ±66） μmol/L] and observation group [（35 ± 12） mg/L vs （131 ± 13） mg/L,（5 ±4） μg/L vs （20 ±5） μg/L,（21 ± 15） ng/L vs （70 ± 19） ng/L,（201 ± 65） μmol/L vs （296 ± 68） μmol/L],and in observation group these indices were significantly lower than those in control group （P 〈 0.05）.Conclusion Early use of ω-3 fish oil fat emulsion can relieve systemic inflammatory response syndrome in patients with septic shock.
