布地奈德/福莫特罗吸入治疗成人哮喘合并COPD的临床效果观察

摘　　要：目的 观察布地奈德/福莫特罗吸入治疗成人哮喘合并慢性阻塞性肺疾病（chronic obstructive pulmonary disease,COPD）临床疗效。方法 选取2014年7月~2015年2月嘉兴市第二医院呼吸内科诊断为哮喘合并COPD急性加重期的成人患者70例,按随机数字表法分为观察组及对照组,每组各35例,观察组在对照组常规治疗基础上加用布地奈德/福莫特罗吸入治疗,对照组患者给予单独应用常规治疗,记录并分析治疗后2组患者治疗效果。结果 观察组患者治疗6个月后肺功能参数包括1秒钟用力呼气容积（forced expiratory volume in one second,FEV1）及呼气高峰流速（peak expiratory flow,PEF）值分别为（72.47±14.04 V/L、226.23±55.93 L/s）,均优于对照组（57.93±13.02 V/L、181.54±40.15 L/s）（P〈0.05）;治疗后观察组细胞因子白介素6（IL-6）值（9.36±1.06）ng/L、肿瘤坏死因子（TNF）为（39.12±8.32）ng/L,分别低于对照组（14.27±2.76）ng/L、（55.36±9.56）ng/L（P〈0.05）;观察组治疗后总有效率为94.29%,高于对照组74.29%（P〈0.05）。结论 布地奈德/福莫特罗吸入治疗成人哮喘合并COPD的疗效肯定、确切,与单独应用常规治疗相比无明显不良反应,能有效改善成人哮喘合并COPD症状,值得临床进一步研究和应用。Objective To explore clinical effect of budesonide/formoterol dry powder inhaler in the treatment of adults with sthma and chronic obstructive pulmonary disease. Methods 70 cases from July 2014 to February 2015,were randomly divided into observation group and control group,35 cases in each group. Observation group on the basis of conventional therapy plus formoterol with budesonide dry powder inhalation therapy, control group were treated with conventional therapy alone, recorded and analysed of therapeutic effect between two groups. Results The patients six months after the lung function parameters including a second forced expiratory volume （FEV）% predicted and peak expiratory flow rate （PEF） values were （72. 47 ±14. 04 V/L, 226. 23 ± 55.93 L/s） higher than the control group （57.93 ± 13.02 V/L, 181.54 ± 40. 15 L/s） after treatment in observation group value of cytokines. IL-6 ：of observation group was 9. 36 ±1. 06 ng/L,TNF：39. 12 ± 8.32 ＊ ng/L respectively lower than the control group （ 14. 27 ± 2. 76） ng/L, （55.36 ± 9.56 ）ng /L （P 〈 0. 05 ）; after the observation group total effective rate was 94. 29% , higher than 74.29%. Conclusion Efficacy of budesonidc/formoterol dry powder inhaler in the treatment adults with chronic obstructive pulmonary disease asthma is recognition, precise, and comparosto conventional treatment alone haewno significant adverse reactions, can effectively improve symptoms, it is worth further research and clinical applications.
