拉米夫定联合阿德福韦对乙型肝炎患者免疫应答与浆细胞样树突状细胞功能变化的研究

摘　　要：目的探讨拉米夫定联合阿德福韦对乙型肝炎患者免疫应答与浆细胞样树突状细胞功能变化的影响。方法收集48例慢性乙型肝炎患者,随机分为对照组和实验组,每组各24例,在常规治疗基础上,对照组患者给予拉米夫定治疗,实验组在对照组基础上给予阿德福韦治疗,治疗结束后,对所有患者的血清AST、ALT、IFN-α水平及外周血CD4+、CD8+T细胞、CD4+/CD8+、p DCs水平进行检测。结果治疗后,与对照组比较,实验组患者的外周血CD4+T细胞及CD4+/CD8+水平较高(P<0.05);实验组患者的血清IFN-α及外周血p DCs水平较高(P<0.05);实验组患者的血清AST、ALT水平较低(P<0.05)。结论拉米夫定联合阿德福韦能够显著提高乙型肝炎患者的免疫应答以及浆细胞样树突状细胞功能,改善肝功能。Objective To analysis the effect of lamivudine combined with adefovir on changes of immune response and plasmacytoid dendritic cell function in patients with hepatitis B. Methods 48 patients who were diagnosed with chronic hepatitis B were randomly divided into experimental group and control group ,24 cases in each group. On the basis of conventional treatment, patients in the control group received larnivudine therapy, patients in the experimental group were treated with adefovir on the basis of the control group, after the treatment, the serum levels of AST, ALT, IFN-ot and peripheral blood CD4 ＋ , CD8 ＋T cells, CD4 ＋/CD8 ＋ and pDCs levels were detected in all patients. Results After treatment, compared with control group, the peripheral blood level of CD4 ＋ T cells and CD4 ＋/CD8＋ were higher in the experimental group, and the difference was statistically significant （P 〈 0. 05 ） ;the serum level of IFN-et and peripheral blood level of pDCs were higher in the experimental group （P 〈 0. 05 ） ;the serum levels of AST and ALT were lower in the experimental group （ P 〈 0. 05 ）. Conclusion The lamivudine combined with adefovir can significantly improve the immune resoonse and the function of olasma cell like dendritic cells in naticnts with hepatitis B, improve liver function.
