摘　　要：目的 观察研究大剂量免疫球蛋白在血小板输注无效预防中的临床疗效。方法 纳入哈尔滨医科大学附属肿瘤医院输血科2013年2月~2014年6月收治的急性白血病出现无效血小板输注的患者68例,按随机数字表法分为观察组和对照组各34例,对照组给予配型血小板注射治疗,观察组在对照组的基础上给予大剂量免疫球蛋白,随访并记录2组患者血小板（PLT）、血小板增加修正值（corrected count increment,CCI）和临床疗效情况。结果 2组患者治疗后PLT均升高,但对照组下降较快,观察组患者治疗后24 h的PLT为（30.16±2.83）×109/L,高于对照组（17.82±2.51）×109/L（P〈0.05）。观察组患者治疗1 h和24 h CCI值分别为（33.28±7.19、25.06±5.41）×109/L,均优于对照组（18.20±3.21、11.82±2.36）×109/L（P〈0.05）。观察组患者治疗后CCI有效率和临床好转率分别为（47.06%、64.71%）,高于对照组（14.71%、38.24%）（P〈0.05）。结论 大剂量免疫球蛋白在血小板输注无效预防中的临床治疗效果确切。Objective To observe the effect of high dose immunoglobulin in the prevention of platelet transfusion. Methods 68 cases with acute leukemia of ineffective platelet transfusions from February 2013 to June 2014 were randomly divided into observation group and control group with 34 cases in each group. The control group received roached platelet transfusion treatment, observation group received large doses of immunoglobulin on the basis of control group, patients were followed up and platelet （PLT） and platelet increase corrected count increment （CCI） were recorded and clinical efficacy was compared. Results The PLT in two groups post - treatment increased, hut decreased rapidly in control group, the PLT in observation group after lh treatment were （30. 16 ± 2. 83） x 109/L, which was higher than control group （ 17. 82 ± 2. 51 ） × 109/L（ P 〈 0. 05 ）. The CCI in observation group after 1 h and 24 h treatment were（33.28 ± 7.19, 25.06 ± 5.41 ） × 109/L, which were better than control group （ 18.20 ± 3.21,11.82 ± 2. 36） × 109/L （P 〈 0. 05 ）. The effective rate of CCI and clinical improvement rate were （47.06% , 64. 71% ）, which were higher than control group （14. 71% , 38.24% ）（ P 〈 0. 05 ）. Conclusion High dose immunoglobulin has an exact effect in the prevention of ineffective platelet transfusion.
