结节性甲状腺肿双源CT能谱分析

摘　　要：目的研究结节性甲状腺肿双源CT能谱曲线及碘图特点,为鉴别甲状腺占位类型提供依据。方法选择临床手术确诊为结节性甲状腺肿患者35例(结节性甲状腺肿组),正常成人甲状腺70例(对照组),均采用双能量扫描,分别利用liver VNC和Monoenergetic软件得到碘图及能谱曲线,分析与比较两组甲状腺能谱曲线斜率、碘图特点。结果结节性甲状腺肿组男性和女性能谱曲线斜率、碘浓度比值分别为(4.732 5±2.397 7)、(0.420 8±0.212 9);(5.651 0±2.476 0)、(0.431 4±0.213 8),男性和女性能谱曲线斜率、碘浓度比值比较均无统计学差异;对照组男性和女性甲状腺能谱曲线斜率、碘浓度比值分别为(3.432 1±1.361 7)、(0.520 8±0.312 8);(4.611 4±2.173 0)、(0.476 6±0.321 8),男性和女性甲状腺能谱曲线斜率、碘浓度比较差异均无统计学意义;结节性甲状腺肿组和对照组甲状腺能谱曲线斜率分别为(5.255 0±2.408 0)、(9.816 4±1.860 0),结节性甲状腺肿组能谱曲线斜率明显小于对照组,差异有统计学意义(P<0.05);结节性甲状腺肿组和对照组甲状腺碘浓度比值分别为(0.426 7±0.267 1)、(0.796 5±0.749 4),结节性甲状腺肿组碘浓度比值明显小于对照组,差异有统计学意义(P<0.05)。结论结节性甲状腺肿能谱曲线及碘浓度比值有一定特征性,且与正常成人有差异,对于鉴别甲状腺占位类型有重要指导价值。Objective To study the nodular goiter dual source CT spectral curve and the features of iodine map,and provide the basis for the identifying the type of thyroid gland.Methods 35 patients diagnosed as nodular goiter were selected as the case group.70 normal adult＇s thyroids were set as control group.All the thyroids of the research objects were scanned with dual energy.Iodine map and energy spectrum curve were obtained by liver VNC and Monoenergetic software.The thyroid energy spectrum curve slope and iodine figure characteristics of two groups were analyzed and compared.Results In case group,the energy spectrum curve slope and iodine concentration in male and female respectively were（4.732 5±2.397 7）,（0.420 8±0.212 9）;（5.651 0±2.476 0）,（0.431 4±0.213）8.There were no significant difference between male and female;in normal group the energy spectrum curve slope and iodine concentration respectively in male and female were（3.432 1±1.361 7）,（0.520 8±0.312 8）;（4.611 4±2.173 0）,（0.476 6±0.321 8）.There were no significant difference between male and female;Nodular goiter and normal adult thyroid energy spectrum curve slope were（5.255 0±2.408 0）and（9.816 4±1.860 0）.There were significant difference.Nodular goiter and normal adult thyroid iodine concentration were（0.426 7±0.267 1）and（0.796 5±0.749 4）,and there were no difference.Conclusion Spectrum curve and iodine concentration have certain characteristic in nodular goiter,which are different from those of normal adult.It is an important guiding value for the future identification of thyroid occupying type.
