可降解泪小管塞栓应用于中重度干眼患者的疗效观察

摘　　要：目的探讨可降解泪小管塞栓应用于中重度水样液缺乏性干眼症（aqueousteaYdeficien．cy，ATD）患者的临床效果。方法对2013年4月至2014年2月在长春爱尔眼科医院就诊的29例（29只眼）患有中重度ATD患者应用可降解泪小管塞栓，中度患者26例，重度患者3例，随访1周至8个月，观察术后患者自觉症状、泪液分泌及存留情况、荧光素眼表染色情况（包括泪膜破裂时间）、泪河弯曲面的曲率半径及溢泪情况。结果患者自觉眼部干涩、异物感基本消失，泪膜破裂时间延长，荧光素眼表染色均阴性，泪河弯曲面的曲率半径均大于0．5mm，泪液存留不同程度增多，仅1例患者术后1周出现溢泪情况，给予取出泪小管塞栓，建议手术治疗。结论可降解泪小管塞栓的应用，对于中重度ATD患者有一定的帮助，可以暂时或永久性地增加泪液的存留，是一种有效的、简便快捷的延长泪液在眼表的停留时间的方法。Objective To discusses the clinical effect of biodegradable tear small pipe plug bolt in moderately severe watery fluid lack of dry eyes （aqueous tear deficiency, ATD）. Methods On 29 patients （29 eyes） with moderately severe ATD used biodegradable lacrimal pipe plug bolt, 26 pa- tients with moderate, 2 patients with severe, Follow-up of 1 week to 8 months. The postoperative pa- tients＇ subjective symptom, tear secretion, saved the situation, ocular surface fluorescein staining （in- cluding appears burst time）, tears on the surface of the fiver bend radius of curvature and the over- flow of tears were observed. Results Conscious patients with dry eye, foreign body sensation al- most disappeared, it appeared burst time extended, ocular surface fluorescein staining were negative, tears on the surface of the river bend radius of curvature were greater than 0.5 mm and kept differ- ent degree increase, weeks postoperatively in patients with only 1 case of overflow of tears, re- moved out tears small pipe plug bolt and recommended surgery. Conclusions The application of biodegradable lacrimal pipe plug bolt for patients with moderately severe ATD has certain help, can be temporarily or permanently increase the tear to be kept and it is an effective, simple and quick extension of the tears in the eyes of the residence time of approach.
