MDIG在诊断良、恶性胸水中的临床价值

摘　　要：目的结合癌胚抗原(CEA)、乳酸脱氢酶(LDH)探讨胸水矿物性粉尘诱导基因(MDIG)蛋白,对良、恶性胸水诊断的临床意义。方法采用酶联免疫法测定36例恶性胸水及39例结核性胸水的细胞数、淋巴细胞比例、总蛋白、MDIG蛋白、CEA、LDH水平。结果恶性胸水组的年龄、MDIG蛋白、CEA水平高于结核性胸水组(P<0.05)。敏感性分别为93.1%、69.0%、82.8%;特异性分别为79.5%、82.1%、97.4%。联合检测敏感性可达97.2%,特异性可达99.9%。结核性胸水组淋巴细胞比例、总蛋白水平高于恶性胸水组(P<0.05)。敏感性分别为84.6%、94.9%;特异性分别为89.7%、51.7%。联合检测敏感性为99.2%,特异性为95.0%。结论胸水中的MDIG蛋白是诊断恶性胸水的重要参考指标。年龄、MDIG蛋白、CEA对诊断恶性胸水有重要临床价值。胸水淋巴细胞比例及总蛋白对诊断结核性胸水有重要临床价值。Objective To investigate the clinical significance and diagnostic value of MDIG protein, combined with CEA and LDH, in differential diagnosis between benign and malignant hydrothoraxs. Methods The cell counts, lymphocyte proportion ,the levels of total protein, MDIG protein, CEA and LDH were detected by ELISA in 36 patients with malignant hydrothorax （malignant hydrothorax group）, and 39 patients with tuberculous hydrothorax （tuberculous hydrothorax group）. Results The patient＇ s age, the levels of MD!G protein and CEA in malignant hydrothorax group were significantly higher than those in tuberculous hydrothorax group （ P 〈 0. 05 ）, and the ＂detection sensitivity was 93. 1%, 69%, 82. 8%, respectively and detection specificity was 79.5%, 82.1%, 97.4%, respectively. However the sensitivity and specificity of combined detection were 97.2% and 99.9%, respectively. The total protein levels and lymphocyte proportion in tuberculous hydrothoraxs group were significantly higher than those in malignant hydrothorax group （ P 〈 0.05）, and detection sensitivity was 84.6%, 94.9%, respectively and detection specificity was 89.7%, 51.7%, respectively. However the sensitivity and specificity of combined detection were 99.2% and 95.0%, respectively. Conclusion MDIG protein plays an important role in the diagnosis of malignant hydrothoraxs. The patient＇ s age, MDIG protein and CEA have important clinical significance in diagnosis of malignant pleural effusion, furthermore, lymphocyte proportion and total protein levels are of important clinical significance in the diagnosis of tuberculous hydrothoraxs,
