精蛋白锌重组赖脯胰岛素混合注射液(50R)强化治疗初诊2型糖尿病疗效观察

摘　　要：目的探讨初诊2型糖尿病患者应用精蛋白锌重组赖脯胰岛素混合注射液(50R)(优泌乐50)强化治疗的效果和用药依从性。方法 72例初诊2型糖尿病患者随机分为对照组37例和观察组35例。对照组于三餐前15min皮下注射门冬胰岛素,晚22:00皮下注射甘精胰岛素;观察组三餐前15min皮下注射优泌乐50;2组均连续用药14d。检测2组治疗前、治疗14d后空腹血糖(fasting plasma glucose,FPG)、餐后2h血糖(2-hour plasma glucose,2hPG);出院后随访2个月,记录2组自行停药患者比率。结果观察组治疗14d后FPG[(6.54±0.81)mmol/L]、2hPG[(9.66±1.24)mmol/L]较治疗前[(12.22±3.91)、(17.87±3.47)mmol/L]降低(P<0.05);对照组治疗14d后FPG[(6.43±0.73)mmol/L]、2hPG[(9.27±1.18)mmol/L]较治疗前[(12.52±4.03)、(17.82±3.54)mmol/L]降低(P<0.05);2组治疗14d后FPG、2hPG水平比较差异无统计学意义(P>0.05);观察组胰岛素日用量与对照组比较差异无统计学意义(P>0.05);出院后2个月,对照组自行停药患者比率(40.5%)高于观察组(17.1%)(P<0.01)。结论初诊2型糖尿病患者应用优泌乐50强化治疗血糖控制效果满意,患者出院后治疗依从性较高。Objective To discuss the therapeutic effect of intensification therapy by insulin lispro mix 50/50 on the preliminarily diagnosed type 2 diabetes mellitus（T2DM）and its compliance.Methods Seventy-two patients with preliminarily diagnosed T2DM were randomly divided into control group（n=37）and observation group（n=35）.Control group received subcutaneous injection of insulin aspart 15 min before three meals and subcutaneous injection of insulin glargine at 22：00.Observation group received subcutaneous injection of insulin lispro mix 50/50 before three meals.Both two groups were treated for 14 days.The fasting plasma glucose（FPG）and 2-hour postprandial glucose（2hPG）were detected before and after 14-day treatment in both two groups.All patients were followed up for 2 months after discharge,and the proportions of drug withdrawal patients were recorded in two groups.Results The levels of FPG（（6.54±0.81）mmol/L）and 2hPG（（9.66±1.24）mmol/L）after 14-day treatment were significantly lower than those before treatment（（12.22±3.91）,（17.87±3.47）mmol/L）in observation group（P〈0.01）.The levels of FPG（（6.43±0.73）mmol/L）and 2hPG（（9.27±1.18）mmol/L）after 14-day treatment were also significantly lower than those before treatment（（12.52±4.03）,（17.82±3.54）mmol/L）in control group（P〈0.05）.There were no significant differences in the levels of FPG and 2hPG after 14-day treatment between two groups（P0.05）.There was no significant differences in the daily dosage of insulin between observation group and control group（P〉0.05）.The proportion of drug withdrawal patients was significantly higher in control group（40.5%）than that in observation group（17.1%）（P〉0.01）.Conclusion Insulin lispro mix 50/50 is effective for controlling plasma glucose in patients with preliminarily diagnosed T2DM and the compliance is fine after discharge.
