摘 要:目的探讨Th22细胞在免疫相关性全血细胞减少症(IRP)患者外周血中的变化及意义。方法选择2013年12月至2015年3月潍坊医学院附属医院及潍坊市人民医院住院IRP患者45例为研究对象,其中初诊组22例,复诊治疗有效组(血常规恢复正常)18例,治疗无效组(均为造血衰竭患者)5岁,同期12名健康体检者设为对照组。应用流式细胞术检测Th22细胞在各组外周血中所占比例,ELISA法检测白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)和转化生长因子β(TGF-β)在各组外周血中的表达水平。结果 IRP初诊组、治疗无效组和治疗有效组Th22细胞比例、IL-6、TNF-α的表达水平均高于健康对照组(均P<0.05);IRP治疗无效组高于初诊组(P<0.05),IRP初诊组高于IRP治疗有效组(P<0.05)。在IRP治疗有效组,Th22细胞数量与缓解(血常规恢复正常)时间呈正相关。IRP治疗无效组、初诊组和治疗有效组TGF-β的表达水平均低于健康对照组(均P<0.05),且治疗有效组高于初诊组(P<0.05),初诊组高于治疗无效组(P<0.05)。结论外周血Th22细胞数量的增多与IRP的发生、病情进展可能呈正相关,且IRP患者细胞因子IL-6、TNF-α表达水平的升高及TGF-β的表达水平下降在一定程度上促进了Th22细胞的分化发育。Objective To explore the change of Th22 cells of patients with immune-related pancy- topenia(IRP) in peripheral blood and evaluate its significance. Methods A total of 45 IRP patients hospitalized in the Affiliated Hospital of Weifang Medical College and Weifang People ' s Hospital from December 2013 to March 2015 were recruited and divided into 3 groups. IRP pre- therapy(n = 22), the effective (n = 18 ), the ineffective (n = 5 ), and healthy donor controls (n = 12 ) were selected. The proportion of Th22 cells in peripheral blood of each group was evaluated by flow cytometry. The transforming growth factor-^(TGF-^), tumor necrosis factor-a(TNF-α)and interleukin-6(IL-6)were measured by ELISA. Results The percentage of Th22 cells and the lev- el of TNF-a and IL-6 in groups IRP pre-therapy(the effective and the ineffective)were significant- ly higher than that in control group(P〈0.05). The percentage of Th22 ceils and the level of TNF-α and IL-6 in IRP pre-therapy group was lower than that in ineffective group(that from low to high respectively-the effective group, IRP pre-therapy group and ineffective group, all P〈 0.05). However, level of TGF-β in groups IRP pre-therapy(the effective and the ineffective)were significantly lower than that in control group(that from high to low respectively, the effective group, IRP pre-therapy group and ineffective group, all P〈0.05). Conclusion The number of Th22 cells is elevated in IRP sufferers. Th22 cells may be positively related with the progress of IRP. And an increasing level of TNF-α and IL-6 promotes the multiplication of Th22 cells. A lower level of TGF-β weakens the suppression to proliferation of Th22 cells.
