摘 要:【目的】初步研究超声检测梗阻性黄疸(OJ)大鼠肾脏血流动力学改变的特点及其价值。【方法】健康雄性SD大鼠40只,随机分为实验组30只(分为6批,每批5只),对照组10只(分为2批,每批5只)。实验组结扎胆总管建立OJ模型,对照组仪以开腹-关腹处理。两组大鼠分别于术后d7及d14麻醉状态下予以多普勒超声检测其三级肾动脉(肾主动脉、段动脉、叶间动脉)血流动力学参数:收缩期最大峰值流速(PSV)、舒张期最小流速(EDV)、阻力指数(RI);并于超声检测完后予以取血检测血液生化指标:血清尿素氮(BUN)、肌酐(Cr)及血清直接胆红素(DB)、总胆红素(TB)。比较两组血液生化指标、肾动脉血流动力学参数PSV、RI的变化。【结果】实验组BuN、Cr及DB、TB随着时间延长明显增高,并且d7、d14数据均高于对照组;BUN、Cr两组间数据比较,仅d14数据有统计学意义(P〈0.05);DB、TB两组间数据比较,d7、d14数据均具有统计学意义(P〈0.05)。血流动力学参数两组间数据比较,实验组较对照组各级肾动脉PSV减低,RI增高,并且随着时间延长而更加明显,除两组间d7肾主动脉RI比较无统计学意义,余两组间d7、d14数据比较均具有统计学意义(P〈0.05)。【结论】超声检测OJ大鼠肾脏动脉PSV、RI值较对照组大鼠有明显改变,提示OJ大鼠存在肾脏损伤,并且其血流动力学改变的出现早于血液生化所示肾功能异常;超声检测OJ大鼠肾动脉血流动力学对肾脏损伤有一定的评估和预测作用,且早于血液BUN、CR所示肾功能异常。[Objective]To study the changes of renal hemodynamics in rats with obstructive jaundice (O J) under ultrasound and to observe effects on different values. [Methods]Forty healthy male SD rats were randomly divided into experimental group ( n =30 then they were further divided into 6 groups, each group having 5). Meanwhile, the control group consisted of the remaining 10 (2 groups with 5 in each group). The experimental group was treated by ligation of the common bile duct to build the model of obstructive jaundice, and the control group was treated with sham operation. Both groups of rats were given postoperative 7 d and 14 d ultrasound that showed three different grades renal artery (the renal artery, the segmental artery, and the inter-lobar artery). Blood flow parameters of peak systolic velocity (PSV), minimum diastolic velocity (EDV), resistance index (RI), blood serum urea nitrogen (BUN), creatinine (CR), serum direct bilirubin (DB), and total bilirubin (TB) were tested after the ultrasound test.By comparing the two groups data for blood biochemical parameters and renal artery hemodynamic parameters, the following results were concluded.[Results]The BUN, DB, Cr, and TB in the experimental group were higher than in the control group, and these levels significantly increased with the passage of time. The 7 d values of Cr and BUN were not statistically significant, but the 14 d values were statistically significant ( P 〈0.05). DB and TB data were also statistically significant ( P 〈0.05). PSV of the renal artery in the experimental group was decreased, RI increased, and all other values significantly increased with the passage of time. Except for 7 d renal artery RI, all of other values changed with statistical significance ( P 〈0.05). [Conclusion] PSV and RI of the renal artery in rats with obstructive jaundice have changed significantly, Which indicates that the renal injury in rats with obstructive jaundice, and the change of renal hemodynamics is
