改良冰敷法在前牙引导骨组织再生同期种植患者中的应用

摘　　要：目的评价改良冰敷法在前牙引导骨组织再生(Guided Bone Regeneration,GBR)同期种植患者中应用的效果。方法将符合标准的82例前牙GBR同期种植的患者,按就诊单双号随机分成对照组和干预组各41例,对照组给予清水冰袋以传统方法于术后24h内根据术区肿痛、出血情况间断自行冰敷。干预组使用15%乙醇冰袋根据冰敷时间表进行冰敷。即术前给予自制15%乙醇冰袋术区冰敷15min后再手术,手术后立即冰敷3h,并于术后第1、2、3天进行1h冰敷。对两组在手术时间、术后出血、疼痛、面部肿胀度分级分别进行评价。结果干预组手术时间显著短于对照组,术后第1天术区出血显著少于对照组,术后不同时间干预组面部肿胀、疼痛程度显著轻于对照组(均P<0.01)。结论改良冰敷可以缩短前牙GBR同期种植患者的手术时间,减轻术后出血、疼痛、肿胀反应,对促进患者术后恢复有良好的效果。Objective To evaluate the effect of applying modified cold therapy in patients receiving guided bone regeneration and immediate dental implantation in the anterior teeth region.Methods Eighty-two patients to receive guided bone regeneration（GBR）and immediate dental implantation in the anterior region were randomly divided into a control group and an intervention group,41 cases of each group.The control group was given cold compress with water icebag at the operation site 24 hours after operation according to the swelling and hemorrhage,while the intervention group was provided with a modified cold therapy：preoperative ice compress with homemade 15% alcohol ice pack for 15 minutes,ice compress for 3himmediately after the surgery and for 1hon the 1st,2nd and 3rd day after the surgery.The operation time,postoperative bleeding,pain and swelling were evaluated for both groups.Results The operation time of the intervention group was significantly shorter than the control group;on the first day after the operation,hemorrhage of the former was significantly less than the latter;moreover,at different time points after the operation,the swelling and pain degree of the intervention group were significantly milder than the control group（P〈0.01 for all）.Conclusion The modified cold therapy can shorten the operation time and reduce postoperative bleeding,pain and swelling in patients undergoing guided bone regeneration and immediate dental implantation in the anterior teeth region,which can promote their postoperative recovery.
