综合医院老年病区医养结合运行机制实践

摘　　要：目的探讨综合医院老年病区医养结合的运行机制及实践效果。方法在老年病区（5个护理单元,236张床位）实施医养结合运行机制,每个护理单元设住院医疗（医）和托护养老（养）床位,医生根据患病老人病情变化和康复进程进行动态评价及医养互换。统计2013、2014年276例、290例入住患者的相关指标。结果 2年均未发生用药错误;2013、2014年分别发生坠床、非计划脱管等相关安全事件24、16例次,医院感染发生率6.21%-9.78%,患者满意率分别为92.51%、97.36%,床位使用率在98%以上,病区收支结余2年合计为105 202元。结论老年病区实施医养结合运行机制,较好地解决了患病老年人医疗及养护问题,社会满意度高。但资源偏紧张的问题仍然存在,需全社会共同努力来不断完善。Objective To explore the operating mechanism and practice effect of medical-nursing combination in geriatric ward of general hospital.Methods We implemented medical-nursing combined operating mechanism in geriatric ward（5nursing units,236beds）,and each nursing unit was set up with medical treatment（treatment）and nursing care（nursing）beds,then physician made dynamic evaluation and medical-nursing exchange according to illness changes and recovery process of the elderly patients.The related indicators were calculated for 276 and 290hospitalized patients in 2013 and 2014.Results There was no medication error in the2 years.In 2013 and 2014,drop of bed,unplanned extubation and other related security incidents were 24 and 16case-times,the incidences of nosocomial infection were 6.21%-9.78%,patients′satisfaction rates were 92.51% and 92.51%,bed utilization rates were above 98%,the total balance of payment surplus of the ward in the 2years was 105 202 yuan.Conclusion The medical-nursing combined operating mechanism in geriatric ward could solve the medical and nursing problem for elderly patients well,and the satisfaction degree of the society is high.But the tight resource is still a problem,which need the whole society work together to improve continuously.
