危重症患儿院内安全转运流程化管理

摘　　要：目的探讨流程化管理在危重症患儿院内安全转运中的作用,保障患儿转运安全。方法选择2012年1月至2013年1月儿科急诊院内转运的626例危重症患儿为对照组,2013年2月至2014年1月院内转运的758例危重症患儿为观察组,对照组采用常规方法转运,观察组采用流程化管理方法转运,包括制定危重症患儿院内安全转运制度及流程,加强转运前、转运中及转运后的流程化管理等。比较两组患儿一次转运成功率及转运不良事件发生率。结果观察组均一次转运成功,对照组621例一次转运成功,5例实施二次转运成功,观察组一次转运成功率显著高于对照组(P<0.05)。观察组转运发生不良事件21例,对照组为40例,两组比较,差异有统计学差异(P<0.01)。结论将流程化管理应用于危重症患儿院内转运的管理与实践中,使转运医疗行为制度化、流程化,能够有效降低转运不良事件发生率,规避转运风险,为患儿安全转运提供保障。Objective To evaluate the effect of processed management in intrahospital transport for critically ill children,and to ensure the safety of children transport.Methods Totally 626 critically ill children who were transferred out of the emergency department during January 2012 to January 2013 were selected as a control group,and other 758 children who were transferred during February 2013 to January 2014 were perceived as an observation group.The control group was transferred using conventional method,while the observation group was transferred utilizing processed management,which including developing transport systems and processes,and strengthening processed management before,during and after transport.The rates of one-time success transport and adverse events during transport were compared between the two groups.Results All children in the observation group were successfully transferred only one time,while it was 621 in the control group,and the other 5children were transferred for a second time,the rate of one-time success transport in the observation group was significantly higher than that in the control group（P〈0.05）.Totally 21 children in the observation group experienced adverse events,and it was 40 in the control group,there was significant difference in the two groups（P〈0.05）.Conclusion Processed management in intrahospital transport for critically ill children makes transfer institutionalized and processed,reduces the rate of adverse events,and avoids the risk of tranport,then provides gurantee for safe transport.
