“动-定序贯八法”中医辨治对早期2型糖尿病患者胰岛素抵抗及血清成纤维细胞因子21水平的影响

摘　　要：目的:观察"动-定序贯八法"中医辨治对早期2型糖尿病患者胰岛素抵抗及血清成纤维细胞因子21水平的影响。方法:将纳入研究的58例患者随机分为两组,对照组28例,治疗组30例,两组均给予统一的生活方式干预,对照组加口服西药治疗,治疗组给予"动-定序贯八法"中医辨证论治,疗程3个月。观察患者中医证候积分、体重指数、腰臀比、血糖、Hb A1c、HOMA-IR及FGF-21水平的变化。结果:两组总体疗效相当,治疗后两组BMI、WHR、FPG、2h PPG、Hb A1c、HOMA-IR及血清FGF-21水平均显著下降(均P<0.01),但两组间比较无差别;治疗后两组中医证候总积分均较治疗前显著下降(P<0.01),且治疗组较对照组下降更为明显(P<0.01)。结论:"动-定序贯八法"中医辨治早期2型糖尿病疗效确切,并可改善患者的胰岛素抵抗水平,其机制可能与FGF-21有关。Objective： To observe the effects of pure Chinese medicine which is based on the variable-stable and sequential eight methods theory on insulin resistance and serum FGF-21 levels in patients with early type 2 diabetes mellitus.Methods： 58 patients with early T2 DM were divided into 2 groups： 28 patients in the control group were administrated with Metformin hydrochloride sustained-release tablets（ 0.5 g,qd or bid） and 30 patients in the therapy group were administrated with pure Chinese medicine based on the variable-stable and sequential eight methods theory.All of the objects were given the same diet and exercise guidance.The observed period were 3 months.Total score of syndrome integral was accumulated and BMI values were recorded and FPG,2 h PPG,FINS,Hb A1 c,serum FGF-21 levels and HOMA-IR were measured.Results： The final effect had no obvious difference between the two groups（ P〈0.05）.After treatment,BMI,WHR,FPG,2 h PPG,Hb A1 c,HOMA-IR and serum FGF-21 levels in the two groups were all decreased when compared to those pretherapy（ all P〈0.01）,but there was no difference between the two groups.At the same time,TCM syndrome integral scores were alleviated（ P〈0.01） and the effect was more distinctly in therapy group（ compared to that of the control group P〈0.01）.Conclusion： Early T2 DM patients can be treated exactly by the pure Chinese medicine of variable-stable and sequential eight methods via ameliorating their insulin resistance the mechanism may be related with FGF-21.
