温阳和营、凉血化瘀法配合复方甘草酸苷治疗寻常型银屑病疗效及对血清TNF-α影响

摘　　要：目的观察温阳和营、凉血化瘀法配合复方甘草酸苷注射液治疗寻常型银屑病的临床疗效及对血清TNF-α的影响。方法将98例寻常型银屑病患者随机分为2组,对照组给予复方甘草酸苷注射液静滴,1次/d,连续12 d,配合维A酸乳膏外涂(1次/d)+0.05%卡泊三醇软膏外涂(2次/d),4周为1个疗程;试验组在静滴复方甘草酸苷注射液的基础上给予温阳和营、凉血化瘀中药口服,4周为1个疗程。观察2组治疗1个疗程后临床疗效及治疗前后银屑病皮损面积和严重程度指数(PASI)评分、血清TNF-α水平变化情况。结果试验组总有效率明显高于对照组(P<0.05);2组治疗后PASI评分和血清TNF-α水平均较治疗前明显降低(P均<0.05),且试验组明显低于对照组(P均<0.05)。结论温阳和营、凉血化瘀法配合复方甘草酸苷注射液能有效缓解寻常型银屑病患者临床症状,降低血清TNF-α水平,值得临床深入研究。Objective It is to observe the curative effect of Warming Yang and Harmonize the Nutrient,Cooling Blood and Removing Blood Stasis method combined with the Compound Glycyrrhizin in the treatment of psoriasis vulgaris and the influence on the serum TNF- α level. Methods 98 patients with psoriasis vulgaris were divided into observation group and control group according to random number table method and 45 cases in each group. The patients in the control group were treated with Compound Glycyrrhizin injection 20 m L for intravenous drip,once a day for 12 days,with external application of vitamin A acid cream once a day and 0. 05% Calcipotriol Ointment twice a day,4 weeks for one course of treatment. The patients in the observation group were treated with Warming Yang and Harmonize the Nutrient,Cooling Blood and Removing Blood Stasis method combined with Compound Glycyrrhizin injection,4 weeks for one course of treatment. The clinical curative effect and the changes of PASI score and serum TNF- α were observed after treatment. Results The total effective rate of the observation group was significantly higher than that of the control group（ P 0. 05）. The PASI score and the serum TNF- α were significantly reduced after treatment（ all P 0. 05）,and the reduction of observation group was more obvious than that of control group（ all P 0. 05）. Conclusion Warming Yang and Harmonize the Nutrient,Cooling Blood and Removing Blood Stasis method combined with Compound Glycyrrhizin can effectively relieve the clinical symptoms of patients with psoriasis vulgaris,and reduce the level of serum TNF- α,it is worthy of clinical research.
