小剂量左西替利嗪间歇疗法结合中医体质个体化治疗预防慢性荨麻疹复发的研究

摘　　要：目的观察小剂量左西替利嗪间歇疗法结合中医体质个体化治疗预防慢性荨麻疹复发的疗效及长期用药的安全性。方法所有患者在急性发作期先予H1和H2受体拮抗剂联合治疗,待症状和体征完全改善后,根据患者不同中医体质分为特禀质、湿热质、痰湿质、气虚质、平和质、瘀血质、阴虚质、阳虚质和气郁质组,每组再随机分为西医组和中西医结合组2个亚组,西医组给予小剂量左西替利嗪间歇治疗(2.5 mg,隔天或每3 d或每周给药1次),中西医结合组在小剂量左西替利嗪间歇治疗的基础上结合中医体质予不同的中药治疗。在治疗的第3,6,12,24个月分别进行随访。结果随着治疗时间的推移,各组的复发率均明显降低,第6,12,24个月各中医体质组中中西医结合组复发率均明显低于西医组,且复发时症状和体征积分均明显低于西医组,差异均有统计学意义(P均<0.05)。治疗后2组未见不良反应事件发生。结论小剂量左西替利嗪间歇疗法结合中医体质个体化治疗能有效、长期预防慢性荨麻疹的复发,且安全性好。Objective It is to observe the curative effect of low-dose Levocetirizine intermittent treatment combined with constitution of TCM individualized treatment on the recurrence,and the long-term safty of patients suffering from chronic unticaria（ CU）. Methods According to patients with different TCM constitution,all patients with CU were divided into the great quality group,heat-dampness group,phlegm and dampness group,Qi-deficiency group,normal group,Yin-deficiency group,Yang-deficiency group,blood-stasis group,and Qi-depression group,each group was randomly divided into two subgroups after all of them had been complete recovery in signs and symptoms with the co-administration of H1- antagonists ebastine and H2- antagonists ebastine. The single Western medicine treatment group received a low-dose Levocetirizine intermittent treatment（ 2. 5mg,every two days or every three days or every week）; the integrative group received the combination treatment of low-dose Levocetirizine intermittent therapy and TCM based on different TCM constitution. Follow up was performed at the3 rd,6th,12 th and 24 th months after treatment. Results With the passage of time to treatment,the recurrence rate were significantly lower in each group; and the recurrence rate in all the integrative groups of the 16 th,12th and 24 th month were significantly lower than those in the single Western medicine treatment groups,and the total scores of signs and symptoms in all the integrative groups were significantly lower than those in the single Western medicine treatment groups,there were significant difference（ all P 0. 05）. No adverse events were found in the 2 groups after treatment. Conclusion Low-dose Levocetirizine intermittent treatment combined with constitution of TCM individualized treatment can effectively prevent the recurrence of chronic urticaria in a long time,and the safety is good.
