规律性尿激酶封管对血液透析患者长期留置深静脉导管功能的影响

摘　　要：目的：探讨规律性尿激酶封管对长期留置深静脉导管功能改善的作用以及使用寿命的影响。方法：选取2013年5月~2014年5月我院血液净化中心进行血液透析患者292例,根据封管方式不同分为对照组和观察组,对照组采用常规肝素溶液封管;观察组每周第1次治疗时使用25000 U/ml尿激酶加25 mg/ml肝素溶液封管。两组患者均连续观察12个月。结果：两组不同封管方式对患者凝血功能无明显影响,但观察组在12个月内长期中心静脉导管的功能不良发生率明显低于对照组,其使用时间明显比对照组长,差异有统计学意义（P〈0.05）。结论：规律性尿激酶封管对长期留置深静脉导管功能有明显的改善作用,并能延长其使用寿命。Objective： To discuss and investigate the improvement and service life change of long term indwelling deep venous catheter by the application of regular urokinase sealing tube.Methods： Selected 292 hemodialysis patients who received blood purification in the blood purification center of our hospital were selected from May 2013 to May 2014 and they were divided into two groups according to different tube sealing methods,the control group and the observed group.Conventional heparin tube sealing tube was applied in the control group,while 25000 U/ml urokinase and 25 mg/ml heparin sealing was applied in the observed group for the first time of treatment every week.The two groups were observed for a consecutive 12 months.Results： There was no significant difference in the blood coagulation function between patients of the two groups,however,the dysfunction rate for the central venous catheter in the long term within 12 months of the observed group was significantly lower than that of the control group,and the service life of the central venous catheter was significantly longer than that of the control group（ P〈0.05）.Conclusion： The regular urokinase sealing tube can obviously improve the function of long term indwelling deep venous catheter and can extend its service life.
