两种鼻肠管置管法应用于克罗恩病患者的效果观察

摘　　要：目的：比较床边徒手置入鼻肠管与胃镜引导下置入鼻肠管法应用于克罗恩病患者的有效性及安全性。方法：选取2014年1月~2015年8月我院收治的106例行鼻肠管置入的克罗恩病患者,随机等分为观察组和对照组,对照组采用在胃镜引导下置入鼻肠管法;观察组采用床边徒手置入法,对两组的置管成功率、患者痛苦指数、置管相关并发症、置管费用进行比较。结果：两组的置管成功率差异无统计学意义,床边徒手置入组的痛苦指数及置管费用明显低于对照组,差异均有统计学意义（P〈0.05）,两组患者均未发生严重并发症。结论：床边徒手置入鼻肠管,可显著降低克罗恩病患者置管的痛苦指数、置管费用,保证置管成功率,值得在临床推广。Objective： To compare the effectiveness and safety between the nasointestinal tube insertion by bare- hand at bedside and the nasointestinal tube insertion under the guidance of gastroscope which are both applied in the patients with Crohn＇s disease.Methods： Selected 106 cases of patients with Crohn＇s disease who had underwent the nasointestinal tube insertion in our hospital from January 2014 to August 2015 and equally divided them into the observation group and control group at random.The control group adopted the nasointestinal tube insertion under the guidance of gastroscope,while the observation group adopted the nasointestinal tube insertion by bare- hand at bedside.Compared the tube insertion success rate,patient discomfort index,tube insertion correlated complications and tube insertion fees of the two groups.Results： The difference of the tube insertion success rate between the two groups had no statistic significance,however,the patient discomfort index and tube insertion fees of tube insertion by bare- hand at bedside of the observation group were evidently lower than that of the control group and the difference was of statistic significance（ P〈0.05）.And both the groups had no severe complication occurrence.Conclusion： The nasointestinal tube insertion by bare- hand at bedside can effectively lower the discomfort index,tube insertion fees of the patients with Crohn＇s disease and ensure the tube insertion success rate,which is worthy of clinic popularization.
