脑卒中患者延续护理服务方案的构建研究

摘　　要：目的:探讨脑卒中患者的延续护理服务方案。方法:借鉴延续护理模式作为框架指导方案设计,选择护理措施的实证推荐作为干预内容,形成脑卒中患者的延续护理服务方案。结果:该方案由5周4次系列护理服务活动组成,分出院前和出院后2个阶段。出院前阶段提供住院期健康教育和综合性护理评估;第二阶段提供出院后电话随访、家庭访视或回院随访;除了干预者发起的护理活动,方案还提供患者在需要时可以自由发起的电话咨询。方案实施者是受过培训的专科护士,并与神经科专科医师、康复理疗师、心理咨询师联系以备提供治疗意见和转诊建议。结论:通过前期的小样本临床试验,证实该方案有改善生活质量部分指标及提高患者满意度的积极效果。Objective： To explore the continuing nursing service programs for cerebral apoplexy patients.Methods： Formulated the continuing nursing service program for cerebral apoplexy patients by designing the programs under the guidance and framework of the continuing nursing mode and selecting the recommended evidences of nursing measures as the intervention content.Results： The program consisted of four systematic nursing service activities held in 5weeks,which were divided into the pre- discharge phase and post- discharge phase.The pre- discharge phase provided the evaluation of hospitalization health education and comprehensive nursing; the post- discharge phase provided the telephone follow- up interview,home visit and back- to- hospital follow up interview; except for the nursing activities launched by the intervener,the program also provided the patients with the telephone consultants which could be launched by them freely when they needed.The program implementers were the professionally trained specialty nurses,who kept in contact with the neurological specialty physicians,rehabilitation therapists and psychological consultants so that they could promptly provide treatment opinions and treatment transfer advices for the patients.Conclusion： Through the previous small sample clinic experiment,the program is demonstrated that it owns the positive effect to improve partial life quality index and the patients＇ satisfaction.
