RA患者血清中RF、CCP抗体、GPI抗原以及AKA检测的临床意义

摘　　要：目的探讨类风湿性关节炎(RA)患者类风湿因子(RF)、抗环瓜氨酸肽(CCP)抗体、葡萄糖-6-磷酸异构酶(GPI)抗原和抗角蛋白抗体(AKA)检测的临床意义。方法对120例RA患者(RA组)和150例健康对照组进行RF、CCP抗体、GPI抗原、AKA检测,其中RF采用透射比浊法检测,CCP抗体采用胶体金法检测,GPI抗原采用酶联免疫吸附试验,AKA采用免疫荧光法。结果 RF、CCP抗体、AKA、GPI在RA组的特异性和灵敏度分别为85.1%和71.6%、97.3%和80.0%、96.0%和59.2%、40.8%和60.8%;RA组中RF、CCP抗体和GPI阳性率分别为7.16%、80.1%、60.8%,显著高于对照组的2.0%、0.0%、2.0%,差异有统计学意义(P<0.05);RF+CCP+AKA+GPI联合诊断特异性较高,但是灵敏度较低,明显低于单独检测的抗CCP抗体,差异有统计学意义(P<0.05)。结论联合RF、CCP抗体、GPI抗原以及AKA检测有助于RA的诊断。Objective Toinvestigate the clinical significance of rheumatoid factor（RF）,anti-cyclic citrullinated pepetide（CCP） antibodies,glucose-6-phosphate isomerase（GPI） and antikeratin antibody（AKA） in the diagnosis of rheumatoid arthritis（RA） patients. Methods 105 patients with rheumatoid arthritis were recruited as RA group and 120 cases of healthy people as control group. Serum levels of RF,anti-CCP antibodies,GPI and AKA were detected in two groups.Projection turbidimetry were applied to detect RF,Immune colloidal gold to detect anti-CCP antibodies,Enzyme-linked immunosorbent assay to detect GPI and Immunofluorescence to detect AKA. Results In RA group,the specificities of RF,anti-CCP,AKA and GPI were 85.1%,97.3%,96.0% and 40.8%,respectively; the sensitivities were 71.6%,80.0%,59.2% and 60.8%,respectively.The positive rates of RF,anti-CCP and GPI were higher than those of the control group,and the differences were statistically significant（P〈 0.05）.RF＋CCP＋AKA ＋GPI combined diagnosis had higher specificity and lower sensitivity,and the difference was statistically significant（P〈 0.05）. Conclusions RF,CCP GPI and AKA combined detection was significant in the diagnosis of RA.
