六藤四妙汤治疗急性痛风性关节炎的临床研究

摘　　要：目的观察六藤四妙汤治疗急性痛风性关节炎的临床疗效。方法将60例急性痛风性关节炎患者随机分为2组,对照组30例予美洛昔康分散片治疗,治疗组30例采用六藤四妙汤内服治疗。4周后比较2组治疗前后关节肿痛疼痛视觉模拟评分(VAS)、血尿酸、红细胞沉降率(ESR)、C-反应蛋白(CRP)等指标,并统计疗效及不良反应发生情况。结果治疗组总有效率86.67%,对照组66.67%,2组总有效率比较差异有统计学意义(P<0.05)。2组治疗后关节肿痛VAS评分、血尿酸、ESR及CRP较治疗前均下降(P<0.01),且治疗组治疗后明显低于对照组(P<0.05)。治疗组不良反应发生率6.67%,对照组16.67%,治疗组不良反应发生率明显低于对照组(P<0.05)。结论六藤四妙汤治疗急性痛风性关节炎疗效显著,在改善症状,降低血尿酸、ESR及CRP方面优于美洛昔康分散片,且不良反应少,值得临床推广应用。Objective To observe the clinical effects of Liuteng-simiao decoction on the treatment of acute gouty arthritis.Methods 60 patients with acute gouty arthritis were randomly divided into two groups.30 patients in control group were treated by meloxicam dispersible tablets.30 patients in treatment group were treated by Liuteng-simiao decoction for oral administration.The visual analogue scale（ VAS） of joint swelling and pain,uric acid,erythrocyte sedimentation rate（ ESR） and C-reactive protein（ CRP） before and four weeks after treatment were observed in two groups,and the curative effects and adverse reaction were evaluated.Results The total effective rate in treatment group（ 86.67%） was superior to that in control group（ 66.67%,P〈0.05）.The levels of VAS,blood uric acid,ESR and CRP after treatment were decreased as compared with those in before treatment in two groups（ P〈0.01）,and the decrease in treatment group was lower than that in control group（ P〈0.05）.The adverse effects rate in treatment group（ 6.67%） was obviously lower than that in control group（ 16.67%,P〈0.05）.Conclusion Liuteng-simiao decoction has exact effect on the treatment of acute gouty arthritis,has better effects on improve symptoms,decreased uric acid,ESR and CRP than meloxicam dispersible tablets,with fewer adverse reactions,worthy of clinical application.
