纳米银敷料联合川芎嗪治疗Ⅱ度烧伤30例

摘　　要：目的:观察纳米银敷料联合川芎嗪治疗Ⅱ型烧伤的临床疗效。方法:将60名患者随机分为观察组、对照组各30人。对照组使用磺胺嘧啶银霜均匀涂抹于患处,每日换药,共持续14天。观察组采用纳米银敷料覆盖创面,外用医用消毒纱布包扎,每7天更换次1次,共换敷料2次。此外以80 mg盐酸川芎嗪注射液静脉滴注,1次/d,连用2周。分别在清创前,清创后第7、14、21、28天观察创面愈合率及平均愈合时间,取创面分泌物进行细菌培养,并收集患者血液及24小时尿液,采用石墨炉原子吸收法测量血清银及24小时尿银含量。结果:在第7、14、21天观察组创面愈合率高于对照组,且各时点愈合率比较,差异均有统计学意义(P<0.05)。观察组第7天创面分泌物培养即为阴性,杀菌效果优于对照组(P<0.05),此外观察组血清和尿液中银离子排泄较对照组快(P<0.05),在停药第7天即可恢复至用药之前。结论:纳米银敷料联合川芎嗪可提高Ⅱ度烧伤患者创面愈合率,缩短愈合时间,且有较强及迅速的杀菌能力,体内代谢速率较快。Objective： To investigate curative effects of nano silver dressings combined with ligustrazine in treating deep second-degree burn. Methods： Sixty patients were randomly divided into the observation group and the control group, 30 cases each group. The control group accepted smearing silvadene on to the affected surface evenly,changing the dressing, for 14 consecutive days. The observation group received nano silver dressing covering the wound, bandaging with sterilized gauze, changed once every seven days, changing the dressings twice, and intravenous dripping of ligustrazine hydrochloride injection, 80 mg, once per day, for two weeks. Wound healing rate and average healing time were observed before cleaning the wound, at the seventh, 14 th, 21 st and 28 th day after cleaning the wound, the secretion from the wound were taken and bacterial culture was performed, the blood and 24 hours urine of the patients were collected, the contents of serum silver and 24 hours urinary silver were detected by graphite furnace atomic absorption method. Results： The observation group was higher than the control group in wound healing rate at the seventh, 14 th and 21 st day, the difference showed statistical meaning in the comparison of healing rates at different times（P〈0.05）. Wound secretion at the seventh day in the observation group was negative,sterilizing effects was superior to these of the control group（P〈0.05）, silver ion in the blood and urine was excreted quickly（P〈0.05）, the level of silver ion could restore to normal in seven days after stopping taking the medicine.Conclusion： Nano silver dressing could improve wound healing rate of deep second-degree burn, shorten healing time, it shows strong and rapid sterilizing effects with quick excretion rate in vivo.
