河南首次输入性登革热暴发流行控制的思考

摘　　要：2013年,河南禹州市首次发生输入性登革热暴发,通过启动联防联控机制,开展宣传教育、快速杀灭成蚊和消除蚊媒孳生地等措施,使登革热暴发流行得到有效控制。本次疫情控制存在如下问题:(1)对突发公共卫生事件发生缺乏足够认知;(2)疫情处置专业技能滞后与疫情控制需求反差;(3)疫情处置未能做到全面覆盖不留死角;(4)疫区处置盲目施药污染环境现象严重;(5)环境卫生差为病媒生物提供大量生存条件。思考输入性登革热暴发原因:(1)社会人员流动性增大出入境人员逐年递增是输入性登革热暴发的社会隐患;(2)城市扩张和发展导致媒介孳生场所和范围呈扩大趋势;(3)出国劳务人员卫生素质差并缺乏对传染病的个人防范意识。探讨输入性登革热控制对策:(1)搞好出国人员健康教育及其科普知识宣传;(2)开展入境人员疫情检验检疫监测和媒介监测;(3)搞好环境治理和蚊虫控制;(4)加强登革热科普知识宣传搞好灭蚊做好个人防护;(5)检验检疫部门联合当地疾控部门做好防制技能培训及其药械储备。In 2013, the imported dengue fever outbreak occurred for the first time in Yuzhou city of Henan, by launching the joint-control mechanism, developing health education widely, quickly getting rid of local mosquitoes and eliminating mosquito breeding places, that the dengue fever epidemic was controlled effectively. To review the problem among this outbreak control as follows： （ 1 ） Lack of sufficient cognitive of emergent public health events. （2） Professional skills lag contrast with epidemic outbreak control. （3） Failed to achieve comprehensive coverage without dead ends among disposal of epidemic. （4）To abuse pesticide to make pollution of the environment.（5 ）Poor sanitation provides vector biology with conditions. Thinking of imported dengue fever control： （ 1 ） Social mobility increasing yearly and entry-exit travelers are the social hazard of imported dengue fever outbreak. （2） Urban expansion causes expanding of the breeding grounds and the media scope.（ 3 ）The health education of exported labour and their returning surveillance are needed heavily.To discuss the strategies of imported dengue fever control： （1） To improve the concerned health education and popular science propaganda. （2） To carry out the epidemic monitoring and media monitoring for immigrants. （3） To improve the environmental management and theconcerned mosquito control. （4）The scientific knowledge of dengue fever and mosquito control with personal protection are also needed. （5）The inspection and quarantine Department should combine with the local disease control and prevention departments to make vectors control skills training and the concerned machinery reserves.
