高血压对全膝关节置换术围手术期出血的影响

摘　　要：[目的]研究高血压对全膝关节置换术围手术期出血的影响。[方法]回顾性分析2013年1月~2014年12月于本院关节外科接受全膝关节置换的患者,从中严格筛选符合本研究的病例126例,根据患者有无高血压病史及围手术期血压值分为高血压组和正常组,记录患者性别、年龄、身高、体重、围手术期血压、血红蛋白（Hb）、红细胞压积（Hct）、术后引流量、自体引流血回输量、术后输血量。运用Gross方程计算总出血量、隐性出血量。[结果]高血压组与正常组相比总出血量、隐性出血量差异有统计学意义（P=0.03,P=0.04）,正常组男性与女性相比总出血量、隐性出血量及Hb下降差异有统计学意义（P=0.002,P=0.016,P=0.04）,高血压组男性出血量与女性出血量差异无统计学意义（P=0.735）。[结论]高血压可明显增加全膝关节置换患者围手术期总出血量及隐性出血量。[Objective] To examine the effects of hypertension on perioperative blood loss in patients undergoing primary total knee arthroplasty. [Methods] We retrospectively analyzed the patients undergoing primary total knee arthroplasty in our hospital from January 2013 to December 2014 and choosed 126 patients who met our inclusion criteria. Based on the perioperative blood pressure and the history of hypertension,patients were divided into hypertension group（ n = 54） and normal group（ n= 72）. Data were recorded and compared between two groups,which included gender,age,height,weight,pre and postoperative hemoglobin and hematocrit,blood pressure,drain output,autologous drainage blood transfusion,and transfusion. The amount of total blood loss and hidden blood loss were calculated by the Gross equation. [Results] The total blood loss and hidden blood loss in hypertension group was statistically significantly higher than that in normal group（ P = 0. 03,P = 0. 04）.The difference was statistically significant in gender in normal group（ P = 0. 002,P = 0. 016,P = 0. 04） but was not statistically significant in gender in hypertension group（ P = 0. 735）. [Conclusion] Both the total blood loss and hidden blood loss are higher in patients with hypertension.
