摘 要:目的探讨不同胎龄早产儿早期血清尿素氮、肌酐的变化规律及相关因素。方法对2012年1月至2014年12月广西医科大学第一附属医院新生儿病区收治的早产儿,根据出生时胎龄分为A(28~31周)、B(32~34周)、C(35~37周)三组,每组均30例,共90例入选。记录每组患儿出生后1、3、7 d的血清尿素氮、肌酐水平变化,对结果及相关因素进行分析。结果三组不同胎龄早产儿生后1、3、7 d的尿素氮、肌酐差异均有统计学意义(P均〈0.05)。血清尿素氮在出生后早期有先下降而后上升的趋势,肌酐随着日龄的增加有下降的趋势。血清尿素氮与静脉蛋白摄入量正相关,肌酐与日龄负相关(r值分别为0.479和-0.46,均P〈0.01)。结论早产儿血清尿素氮、肌酐受多种因素影响,血清尿素氮、肌酐作为评价早产儿生后早期肾功能的指标是不充分的。ObjectiveTo study the levels and relative factors of serum urea nitrogen, creatine in preterm infants with different gestational age during the early stage after birth.MethodsWe analyzed the levels of serum urea nitrogen, creatine at day 1, 3, 7 after birth of the preterm infants admitted in the Division of Neonatology of our hospital between January 2012 and December 2014. 90 infants were divided into three group according to their gestational age (GA) at birth, A group (GA 28-31 weeks,n=30), B group (GA 32-34 weeks,n=30), C group (GA 35-37 weeks,n=30). The values of serum urea nitrogen, creatine at day 1, 3, 7 of all cases were recorded and analyzed.ResultsThe levels of serum urea nitrogen and creatine at day 1, 3, 7 after birth showed significantly differences (allP values〈0.05). Serum urea nitrogen first decreased then rose during the first days after birth. Creatine showed a tendency to decline with the increase of day age. The serum urea nitrogen levels were positively correlated with intravenous protein intake, and creatine levels were negatively correlated with age (r value was 0.479 and-0.46 respectively, bothP values〈0.01).ConclusionThe levels of serum urea nitrogen and creatine are affected by a variety of factors during the first days after birth, so it is not sufficient to use them to assess renal function in preterm infants in the early stage after birth.
