摘  要:目的:通过与肱骨前外侧入路手术比较,探讨肱骨前路微创技术联合LCP(锁定加压钢板)治疗肱骨干骨折的疗效分析.方法:选取我院2012-06/2015-04收治并确诊为肱骨干骨折的61例患者,随机分为对照组(n=30)和观察组(n=31).对照组行肱骨前外侧入路切开复位内固定(ORIF)联合LCP固定,观察组行肱骨前路微创钢板接骨术(MIPO)联合LCP固定.对比两组患者的切口长度、手术时间、术中出血量、住院时间、骨折愈合时间、肩肘关节功能(采用DASH评分)、并发症发生率.结果:观察组与对照组的住院时间、肩肘关节功能比较,差异无统计学意义(P>0.05);观察组的切口长度、术中出血量、手术时间、并发症发生率、骨折愈合时间优于对照组,差异有统计学意义(P<0.05).结论:肱骨前路微创技术联合LCP治疗肱骨干骨折具有创伤小、术中出血量低、术后疼痛轻、并发症少、安全可靠等优点,最重要的是,该技术具有不显露桡神经和减少医源性桡神经损伤发生的风险的优点,是治疗肱骨干骨折的有效方法.AIM: Compared with anterolateral approach surgery of the humerus,to discuss curative effect analysis of humeral anterior approach and minimally invasive technique combined with LCP( locking compression plate) in the treatment of humeral shaft fracture. METHODS: A total of 61 patients with humeral shaft fractures who were admitted into our hospital from June 2012 to April 2015,were randomly divided into control group with 30 cases and observation group with 31 cases. The control group were treated with open reduction and internal fixation( ORIF) combined with LCP through humeral anterolateral approach,while the observation group were treated with minimally invasive plate osteosynthesis( MIPO) fixation combined with LCP through humeral anterior approach. And the incision length,operative time,intraoperative blood loss,hospitalization time,fracture healing time,function of shoulder and elbow joint( the DASH score),and incidence rate of complications were compared in two groups.RESULTS: By comparison of the observation group and the control group,the difference in hospitalization time,function of shoulder and elbow joint between two groups was not statistically significant( P〉0. 05). The incision length,intraoperative blood loss,operation time,complication rate,fracture healing time of the observation group were better than those of the control group,and the difference was statistically significant( P〈0. 05). CONCLUSION: Humeral anterior approach and minimally invasive technique combined with LCP in the treatment of humeral shaft fractures has the advantage of less trauma,less intraoperative blood loss,less postoperative pain,less complications,being safe and reliable. It is an effective way in the treatment of humeral shaft fractures due to the advantages of reducing the risk of iatrogenic radial nerve injury or exposing the radial nerve.
