非离子型碘水、碘油行子宫输卵管造影发生对比剂逆流的比较

摘　　要：目的探讨子宫输卵管造影(HSG)检查选择使用碘油和非离子型碘水(碘海醇)造影发生逆流的差别。方法整群选择2010年7月—2013年4月在该院进行检查的508例患者最为研究对象,按照造影剂类型分为碘油和非离子型碘水造影剂两组,统计比较两组研究对象发生造影剂逆流的病例数以及逆流的类型。结果 508例研究对象共计有50患者发生了造影剂逆流,占9.84%,其中采用碘油进行造影的研究组发生逆流的患者33例,占12.94%,采用非离子型碘水造影的研究组发生逆流的患者17例,占6.72%,两组比较差异具有统计学意义(χ2=5.50,P<0.05);采用碘油造影中发生淋巴逆流的有21例,占63.64%,发生静脉逆流的4例,占12.12%,发生混合逆流有8例,占24.24%;采用非离子型碘水造影中发生淋巴逆流的有7例,占41.18%,发生静脉逆流的6例,占35.29%,发生混合逆流有48例,占23.53%;两组比较差异无统计学意义(P>0.05)。结论碘化油的优点是显影图像清晰度较高,缺点是副作用多,非离子型碘水虽然图像清晰度不及碘化油,但是副作用少,因此临床应该根据需要选择合适的造影剂。Objective To investigate and compare the incidence of countercurrent of iodipin with that of non-ionic iodine（iohexol） in hysterosalpingography（HSG）. Methods 508 cases underwent examination in our hospital from July 2010 to April 2013 were selected as the subjects and divided into two groups, iodipin contrast medium group and non-ionic iodine contrast medium group in accordance with the type of contrast medium. And the number of patients with countercurrent of contrast medium and the type of countercurrent in the two groups were counted and compared. Results Of the 508 subjects,there were a total of 50 cases（9.84%） with the countercurrent of contrast medium, including 33 cases（12.94%） with the countercurrent of iodipin, and 17 cases（6.72%） with the countercurrent of non-ionic iodine, the difference in the incidence of countercurrent of contrast medium between the iodipin contrast medium group and non-ionic iodine contrast medium group was statistically significant（χ^2=5.50, P〈0.05）. Of the subjects with iodipin for HSG, there were 21 cases（63.64%）with lymphatic reflux, 4 cases（12.12%） with venous reflux, 8 cases（24.24%） with mixed reflux. Of the subjects with nonionic iodine for HSG, there were 7 cases（41.18%） with lymphatic reflux, 6 cases（35.29%） with venous reflux, 48 cases（23.53%） with mixed reflux. The differences in the number of patients with lymphatic reflux, venous reflux and mixed reflux in HSG between the two groups were not statistically significant（P〈0.05）. Conclusion Compared with the non-ionic iodine,iodipin has the advantage of developing higher resolution image, but with more side effects. In the clinical treatment, a suitable contrast medium should be chosen in accordance with the need.
