二次剖宫产后人流术中即时放置吉妮致美宫内节育器的临床探讨  

Clinical Observation of the Effect of Immediate Postabortal Insertion of the Gene Fiex IN IUD in Women Undergoing Abortion after Two Previous of Cesarean Section

摘　　要：目的探讨妇女二次剖宫产后再次意外妊娠时行人流手术同时放置吉妮致美宫内节育器(Gene Fiexl N)IUD的临床效果,为有二次剖宫产史女性提供高效避孕的方法,避免了再次妊娠。方法回顾性研究2010年1月—2012年5月该院主动要求放置吉妮致美IUD避孕的二次剖宫产史妇女200例。合并妊娠要求行人流术并主动要求避孕的女性100例设为观察组,对照组为有二次剖宫产手术史妇女主动要求放置IUD。临床观察两组患者放置后的不良反应率、持续使用率及带环妊娠率。结果电话随访术后不良反应,其中术后1个月月经量减少观察组为25%、对照组为24%;月经量增多观察组为13%、对照组为11%;经期腹痛观察组为11%、对照组为9%;2年后的持续使用率观察组为94%,对照组为93%。观察组与对照组相比差异无统计学意义(P>0.05)。结论女性二次剖宫产后再次意外妊娠人流术中同时放置吉妮致美IUD与对照组相比,同样具有不良反应少,脱落率低等优点,适合有二次剖宫产史妇女人流术中同时放置。Objective To observe the clinical effects of immediate postabortal insertion of the Gyne Fix IN IUD in women undergoing abortion due to the accidental pregnancy after two previous of cesarean section, so as to provide a highly effective contraceptive method for avoiding the subsequent pregnancy. Methods A review was conducted on 200 women with the history of two times of cesarean section who volunteered to insert the Gyne Fix IN IUD for contraception in our hospital from January 1st, 2010 to May 31 st, 2012. Of them, 100 women with pregnancy who requested to undergo abortion and actively requested contraception were set as the observation group, and the other women with the history of 2 times of cesarean section who actively wanted to insert the Gyne Fix IN IUD for contraception were set as the control group. The incidence of adverse reactions after the insertion, continuous use rate of Gyne Fix IN IUD and the rate of pregnancy with Gyne Fix IN IUD of the two groups were observed clinically. Results The results of telephone follow-up showed that there was no statistically significant difference between observation group and control group in terms of the proportion of patients with decreased menstruation 1 month after surgery（25% vs 24%）, the proportion of patients with increased menstruation 1 month after surgery（13% vs 11%）, the proportion of patients with menstrual pain（11% vs 9%） and continuous use rate of Gyne Fix IN IUD after 2 years（94% vs 93%）（P〈0.05）. Conclusion Immediate postabortal insertion of the Gyne Fix IN IUD is suitable for women undergoing abortion due to the accidental pregnancy after two times of cesarean section, for it has low incidence of adverse reactions and low loss rate and other advantages as the insertion of the Gyne Fix IN IUD used in the control group.
