52例腹部闭合性损伤患者临床疗效分析

摘　　要：目的探讨及研究经皮腹腔穿刺置管引流治疗腹部闭合性损伤的效果。方法随机选取该院普通外科自2013年2月—2015年5月收治的52例腹部闭合性损伤患者,研究行经皮腹腔穿刺置管引流非手术方式对腹部闭合性损伤的治疗效果,采用回顾性方式对临床资料进行分析。结果全部患者经皮腹腔穿刺置管引流后,治疗均取得顺利成功。没有发生进行性出血状况,腹膜炎症状也明显消退,胃肠的功能得到逐步恢复。都没有出现腹腔感染、延迟性肝、脾脏出血、肠漏等并发症。全部患者出院后均进行2个月随访,其生活质量评价中的社会功能、情感功能和躯体功能以及呼吸困难、疼痛评分值分别为(25.81±6.01)、(38.97±5.10)、(32.12±5.38)和(53.21±5.48)、(68.65±6.99)分,均比住院时都有显著的改善,患者出院前和随访2月后生活质量评分对比,其差异具有统计学意义(P<0.05)。结论经皮腹腔穿刺置管引流对腹部闭合性损伤的治疗,可以实现对患者的病情的实时监控,其优点为方法简单、创伤较小、安全,能显著的提升治愈率,非常值得在临床上推广应用。Objective To discuss and research the effect of percutaneous abdominal puncture catheter drainage in treatment of blunt abdominal trauma. Methods 52 cases of patients with blunt abdominal trauma treated in the general surgery in our hospital from February 2013 to May 2015 were randomly selected and the effect of percutaneous abdominal puncture catheter drainage in treatment of blunt abdominal trauma was researched and the clinical data were retrospectively analyzed. Results All patients had a successful treatment after percutaneous abdominal puncture catheter drainage, there was no progressive hemorrhage, peritonitis symptoms also obviously subsided, gastrointestinal function had been recovering gradually, all patients had no complications such as abdominal infection, delayed liver and spleen hemorrhage and intestinal leakage, and all patients were followed up for 2 months after discharge, the life quality evaluation such as social function, emotional function and physical function and dyspnea and pain scores were respectively（25.81±6.01）,（38.97±5.10）,（32.12±5.38）,（53.21±5.48）and（68.65±6.99）points, which were obviously improved compared with those in hospital, the difference between the life quality score before discharge and the life quality score after 2 months of follow-up was statistically significant（P〈0.05）. Conclusion Percutaneous abdominal puncture catheter drainage in treatment of blunt abdominal trauma can realize the real-time monitoring of the patient＇s condition and has the advantages such as easy method, less trauma, safe,and it can obviously improve the cure rate, and it is of great promotion and application value in clinic.
