C型臂X线机下关节注射曲安奈德与玻璃酸钠治疗腰椎小关节源性下腰痛

摘　　要：目的探讨C型臂X线机下关节注射曲安奈德与玻璃酸钠治疗腰椎小关节源性下腰痛的方法及疗效。方法选取2006-05-2012-08间治疗的小关节源性下腰痛患者76例,男41例,女35例,单侧腰部疼痛39例,双侧37例,23例合并不同程度臀部放射痛。年龄42-83岁,平均65.3岁。将患者随机平均分成治疗组及对照组,治疗组均行C型臂X线机下小关节注射曲安奈德与玻璃酸钠;对照组均行C型臂X线机下小关节注射曲安奈德。两组患者3个月内均得到随访,采用视觉模拟评分（visual analogue scvale,VAS）对两组患者治疗前后疼痛强度进行评估,并得出患者疼痛的缓解度（PAR）。结果治疗1个月后治疗组完全缓解26例,明显缓解7例,中度缓解3例,无缓解2例,总有效率94.7%;对照组完全缓解23例,明显缓解9例,中度缓解3例,无缓解3例,总有效率92.1%,两组总有效率无显著差异（P〉0.05）。3个月后治疗组完全缓解23例,明显缓解8例,中度缓解6例,无缓解1例,总有效率97.3%;对照组完全缓解13例,明显缓解6例,中度缓解2例,无缓解17例,总有效率55.2%,两组总有效率差异有显著性（P〈0.05）。结论 C型臂X线机下关节注射曲安奈德与玻璃酸钠治疗腰椎小关节源性下腰痛具有满意的疗效,且疼痛不易反弹复发,是一种良好的治疗方法。Objective To investigate the method and effect of the treatment of low back pain（LBP） of lumbar facet joint with triamcinolone acetonide and sodium hyaluronate under the guidance of C arm X-ray. Methods A total of 76 patients with small joint of LBP from May 2006 to August 2012 were selected,41 cases of male,35 females,39 cases of unilateral lumbar pain,37 cases of bilateral loin pain, 23 cases with different degree hip pain radiating. Aged 42 to 83 years old, average 65.3 years old. The patients were randomly divided into treatment group and control group. The treatment group was treated with C arm X-ray guided triamcinolone acetonide and sodium hyaluronate. Control group was injected with C arm X-ray under the guidance of small joint triamcinolone acetonide. The patients in the two groups were followed up in March, and the pain intensity was evaluated by visual analogue scale（VAS）,and the pain relief（PAR） was obtained.Results After 1 month of treatment,in the treatment group 26 patients were completely relieved,7 cases were relieved, 3 cases were moderate and 2 cases had no response, the total effective rate was 94.7%. In the control group,23 cases were completely relieved,9 cases were relieved,3 cases were moderate and 3 cases had no remission, the total effective rate was 92.1%. Therewas no significant difference in total effective rate between the two groups（P〉 0.05）. After 3 months, the treatment group had complete remission in 23 cases, relieved in 8 cases,moderate in 6 cases,and no response in 1 case, the total effective rate was 97.3%. In thecontrol group,there were 13 cases of complete remission,6 cases were relieved,2 cases were moderate,17 cases had no remission,the total effective rate was 55.2%,the total effective rate of the two groups had significant difference（P0.05）. Conclusion C arm X-ray guide articular injection of triamcinolone acetonide and glass sodium hyaluronate in the treatment of small joint LBP has satisfactory curative effect,and the pain is not easy rebound, it
