二氧化碳点阵激光联合硅酮凝胶治疗剖宫产皮肤瘢痕的疗效分析

摘　　要：目的评价二氧化碳点阵激光联合硅酮凝胶治疗剖宫产皮肤瘢痕的疗效。方法 87例剖宫产皮肤瘢痕的患者,随机分为三组,即二氧化碳点阵激光联合硅酮凝胶治疗组(A组)、二氧化碳点阵激光治疗组(B组)和硅酮凝胶治疗组(C组)。分别比较三组的温哥华瘢痕标度评分、整体疗效及不良反应等。结果治疗后3个月,就A、B两组瘢痕评分相比较,A组优于B组,两组间差异有统计学意义(P<0.05)。6个月后,A组的瘢痕评分显著低于B组和C组(P<0.05)。治疗后6个月,A、B、C组三组总显效率分别为66.7%、40.0%和33.3%,A组显著高于B、C两组(P<0.017),A组中增生性瘢痕和凹陷性瘢痕均明显少于B组和C组,且有显著差异,三组的复发率分别为0.0%、1.4%和3.4%。三组均有不同程度的不良反应,A组不良反应最低且与B组和C组有显著差异(P<0.05)。结论 CO2点阵激光联合硅酮凝胶治疗剖宫产皮肤瘢痕比CO2点阵激光和硅酮凝胶单独治疗效果好,可以明显缩短治疗时间,提高治疗效果,无明显不良反应,是一种安全、有效的治疗方法。Objective To observe the therapeutic effect of CO2 fractional laser associated with silicon ointment on scars of abdominal delivery.Methods 87 patients with scars of abdominal delivery were randomly assigned to three groups：A group（30 patients were treated by CO2 fractional laser associated with silicon ointment）,B group（30 patients were treated by CO2 fractional laser）,C group（27 patients were treated by silicon ointment）.Their Vancouver Scar Scale （VSS）scores,clinical outcomes and adverse side effects were compared respectively.Results 3 months later,in the VSS score,A group had better score than B,which is significant difference（P 〈0.05）.6 months later,A group was significantly lower than B group and C group in the VSS scores（P 〈0.05）.The total marked effec-tive rate of A group was 66.7%,markedly higher than 40.0% in B group and 33.3% in C group（P 〈0.017）.The A group has the lowest Side effects and complications compared with the other two group（P 〈0.05）.Conclusion CO2 fractional laser associated with silicon ointment is the most effective and safest way in the treatment of abdominal delivery scars in the three way,and could shorten hos-pital time.
