摘　　要：目的通过对南昌大学第二附属医院188例小切口体外循环心内直视手术的疗效观察,探讨微创个体化小切口在体外循环心内直视手术的临床研究。方法南昌大学第二附属医院2014-01—2015-07个体化小切口进行体外循环心内直视手术188例(A组)。根据体质量抽取同期南昌大学第二附属医院收治的传统正中开胸心脏病手术患者187例作为对照组。比较两组术中术后指标、治疗效果和住院费用等。结果A组在体外循环时间、主动脉阻断时间、呼吸机辅助呼吸时间等指标与B组比较差异无统计学意义(P>0.05);A组术后胸腔引流血量(83.29±15.33)m L、术后输血量(98.92±19.85)m L、术后住院时间(5.85±1.47)d、住院费用(3.52±1.68)万元均明显少于B组(P<0.05)。结论微创个体化小切口心脏直视手术是一种安全、有效、美观的手术方式,实用性强。Objective To discuss the clinical research to individualized small incision with minimal invasion during CPB open-heart surgery based on the curative effect observation of 188 cases with small incision CPB open-heart surgery at Nanchang University No.2 Affiliated Hospital. Methods 188 sufferers undertaking individualized small incision with minimal invasion CPB open-heart surgery in our hospital from January 2014 to July 2015 were selected as experimental group A.187 sufferers undertaking traditional median sternotomy during the same period in our hospital were selected as control group B according to body mass.The intra-and post-operative parameters, therapeutic effect and hospitalization expenses were compared between the two groups. Results There were no significant differences between group A and group B in terms of cardiopulmonary bypass（CPB） time, aortic cross clamping time, and respiratory machine auxiliary breathing time（ P〉0.05）.The terms of postoperative chest drainage volume（83.29 ±15.33）m L,intra-operative blood transfusion（98.92 ±19.85）m L, postoperative hospitalization time（5.85 ±1.47）d, and hospitalization expenses（35.2 ±16.8） thousand RMB in group A were clearly lower than those in group B（P〈0.05）.Conclusion individualized small incision with minimal invasion CPB open-heart surgery is a safe, effective and beautiful operation method with strong practicability.
