肺部CT评分法在早产儿支气管肺发育不良的诊断应用

摘　　要：目的探讨肺部CT评分法在早产儿支气管肺发育不良(broncho pulmonary dysplasia,BPD)的诊断应用,了解BPD患儿的肺部CT分值与其临床严重程度的关系。方法收集32例确诊为BPD早产儿的肺部CT资料,按临床诊断标准分为轻、中、重三度,CT计分则以支气管肺段为单位,按肺部是否存在肺充气过度及肺实质损伤分为两个类别进行计分(双肺共18个肺段,两类病变合计的总分不超过36分),且不论同一肺段内出现多少种同类别的病变,也仅按发生病变肺段计1分处理,最后合计两类病变的总分作为患儿肺部CT的最终评分,统计分析BPD患儿的最终CT评分与临床严重程度之间的相关性。结果 BPD患儿的肺部CT评分与临床严重程度呈显著相关性(r=0.768,P<0.001)。结论肺部CT评分法能有效的反映出早产儿BPD的临床严重程度。Objective To explore the application of the CT scoring system in diagnosis of BPD（ Broncho Pulmonary Dysplasia） in order to understand the relationship between CT score and clinical severity of BPD.Methods 32 infants diagnosed as BPD were included in the study,and after a CT examination of the lung,clinical severity of BPD was categorized as mild,moderate,severe,The CT score of each patient was the sum of the number of bronchopulmonary segments with hyperaeration and parenchymal lesions（ total of 18 pulmonary segments in the two lungs,total score of two pathological changes no more than 36 points） respectively. Even if several different lesions of the same category were detected within one segment,only one point was added to the score. An analysis of the correlation between final CT score and clinical severity was made accordingly. Results The CT score showed better correlation（ r = 0. 768,P〈0. 001） with the clinical severity of BPD. Conclusion The CT scoring system of the lung can effectively reflect the clinical severity of BPD.
